Implications of Baxter’s Latest Gene Therapy Data June 24, 2015 Zack Fink Wednesday morning, Baxalta Incorporated (BXLT), a soon to be publically traded spin-off of Baxter International Inc. (BAX), presented updated data from an ongoing phase I/II open-label clinical trial assessing BAX335 at the 2015 International Society on Thrombosis and Haemostasis (ISTH) Congress. As a reminder, BAX335 is an investigational gene therapy being developed for hemophilia B and consisting of an AAV2/8 vector carrying the FIX-Padua transgene. I previously wrote about BAX335 after Baxter reported initial data from this trial in February 2015, suggesting then that this first data set was a derisking event for uniQure’s (QURE) hemophilia B program: BAX335 was unable to facilitate >20% FIX activity without immune complications. With today’s updated data from a larger patient population, I wanted to publish my thoughts as they relate to the hemophilia B gene therapy space as a whole. In addition, since I am currently bullish on uniQure, I wanted to explain what these data mean in relation to uniQure's AMT-060. Updated BAX335 Data As of the time of data collection, a total of seven patients have been treated with BAX335: BAX335 Dose Cohorts: Low dose: 2E11 [vc/kg] (n=2) Medium dose: 1E12 [vc/kg] (n=3) High dose: 3E12 [vc/kg] (n=2) Primary Takeaways: No patients have developed FIX inhibitors (immune response against FIX protein) to date. “Some FIX expression was observed in the lowest dose cohort.” “In the [medium] dose cohort, two patients have experienced no bleeds without regular infusions of FIX, and one of these patients has had sustained FIX [activity] levels of 20-25% for 12 months.” “In the highest dose cohort, expression levels have peaked above 50%, though the two patients in this cohort experienced an immune response which has led to decreased FIX expression, with one patient resuming regular FIX infusions.” These two patients were observed to have T-cell mediated immune responses. As shown below, one patient in the middle cohort had sustained 20-25% FIX activity at 52-weeks post treatment, with no bleeding episodes or exogenous FIX infusions required: This one patient reaching a 5% FIX activity threshold without signs of elevated liver enzymes or a T cell immune response is encouraging for BAX335 as a potential therapeutic. Nevertheless, it’s unclear – with current data – if this can be translated to a larger patient population (2 out of 3 patients in the middle cohort experienced no bleeding episodes post-treatment). In addition, it’s unclear – despite high FIX expression – if the high dose of BAX335 will be viable given the T-cell mediated immune responses (resulting in treatment with corticosteroids) observed in both patients in the high dose cohort. Results from one of these patients is below and demonstrates one of the potential negative consequences of T-cell mediated immune responses in AAV gene therapy – loss of transgene expression/activity (this is the same issue Spark Therapeutics (ONCE) apparently had with their previous AAV8 hemophilia B gene therapy discussed in their S-1): Study investigators believe the dramatic decrease in FIX activity is due to the delayed start of corticosteroids; however, a larger sample size will be needed to determine if this is true. What This Means for the Hemophilia B Gene Therapy Landscape These data are promising, but not a home run. So far the investigators have been unable to dose BAX335 as high as hoped without evidence of a T-cell mediated immune response or decreased FIX expression, despite improvements in manufacturing compared to the AAV8 hemophilia B candidate investigated at St. Jude’s. This leaves the door open for improvements from other companies pursuing gene therapies for hemophilia B since this apparent T-cell mediated immune response could be due to manufacturing or capsid characteristics specific to BAX335 (and other AAV8 gene therapies). For example, uniQure just began assessing AMT-060 (AAV2/5 with hFIXco transgene) in a phase I/II trial. As a reminder, uniQure uses an improved baculovirus manufacturing process compared to the triple transfection method used by St Jude’s (recall BAX335 uses improved purification during triple transfection manufacturing to significantly decrease the amount of empty capsids in the final product). UniQure has previously demonstrated the ability to dose an AAV2/5 gene therapy (AMT-021) at doses up to 2E13 [vg/kg] without evidence of elevated liver enzymes or T-cell immune response. In other words, if manufacturing is the limiting aspect, AMT-060 could have an upper hand. In the future, if AMT-060 or other hemophilia B gene therapies do not show superiority to BAX335, there is still room in the hemophilia B treatment landscape for more than one winner. Preexisting neutralizing antibodies to wild-type AAV vectors used in gene therapy are key exclusion criteria for administration of these gene therapies to date. According to Boutin et al, the seroprevalence for healthy individuals is about 40% for both AAV5 and AAV8. Approximately 40% of patients diagnosed with hemophilia B may be ineligible for gene therapy with either an AAV5 or AAV8 vector!Patients who have preexisting neutralizing antibodies against AAV8 simply cannot receive BAX335, and the same for AMT-060 and AAV5. And, patients receiving AAV8 gene therapy that need retreatment cannot subsequently receive an AAV8 gene therapy due to the now-present neutralizing antibodies – retreatment would require a non-AAV8 gene therapy. I’m intrigued by the BAX335 data, which are certainly promising and support continued clinical advancement, but, there’s room for improvement - and room in the treatment landscape for other winners. twitter.com/bioterp/status/6138065619...

Per centrum worden maximaal 15 patiënten met hemofilie B behandeld.www.cogem.net/showdownload.cfm?object... 1.2 Factor IX Factor IX (FIX) is een vitamine K afhankelijke serine protease dat een cruciale rol speelt bij de bloedstolling. Het eiwit wordt van nature geproduceerd in de lever. In het geval van Hemofilie B is er een tekort aan FIX, waardoor een bloedstolsel te langzaam vormt of niet stevig genoeg is. Hierdoor duren bloedingen langer of kunnen er enkele dagen na een trauma of ingreep nabloedingen optreden.1,2,3 1.3 Adeno-associated virus Adeno-associated virus (AAV) behoort tot de familie Parvoviridae en het genus Dependovirus.4 Het is een enkelstrengs DNA virus met een genoom van circa 4,7 kb. Het genoom codeert voor twee genen, rep en cap. Het rep gen codeert voor eiwitten die een rol spelen bij de virusreplicatie, de expressie van de structurele eiwitten en de integratie van het virusgenoom in het genoom van de gastheer. Het cap gen codeert voor de structurele eiwitten, die de virusmantel vormen. De rep en cap genen worden geflankeerd door twee ‘inverted terminal repeats’ (ITR’s), die betrokken zijn bij DNA-replicatie en integratie van het DNA in een chromosoom van de gastheer. Voor succesvolle replicatie van AAV is co-infectie met een helpervirus nodig, zoals een adenovirus of een herpesvirus.4,5,6 Als er geen ander virus in de cel aanwezig is, blijft AAV latent in de celkern aanwezig, in afwachting van infectie door een helpervirus. AAV kan verschillende warmbloedige dieren, inclusief de mens, infecteren.5 Er zijn verschillende species van AAV (AAV-1 - 9) bekend die onder andere verschillen vertonen in gastheerspecificiteit en weefseltropisme.5,7 Infecties met AAV komen wereldwijd en frequent voor (80% van de humane populatie is seropositief voor AAV-2), maar gaan voor zover bekend niet gepaard met ziekteverschijnselen.8 Overdracht van het virus vindt waarschijnlijk plaats via de respiratoire of gastro- intestinale route.4 1.4 AAV5-hFIX Het ggo genaamd AAV5-hFIX is afgeleid van een humaan Adeno-associated virus subtype 2 vector (AAV-2), maar is omhuld door de manteleiwitten afkomstig van het humane AAV subtype 5. Uit onderzoek blijkt dat AAV-5 een sterk lever gericht tropisme vertoond.9 De AAV-2 uitgangsvector mist de rep en cap sequenties, waardoor het virus replicatie-deficiënt is. De ITRs zijn de enige AAV-2 sequenties die nog in de vector aanwezig zijn. Tussen deze ITRs is de hFIX expressiecassette geplaatst. De hFIX expressiecassette bestaat uit de volgende onderdelen: - LP1 enhancer/promoter: bestaande uit segmenten van de ‘human apolipoprotein hepatic control region’ (HCR) en de ‘human alpha-1-antitrypsin’ (hAAT) promoter; - Simian virus 40 (SV40) small t antigen intron: ter bevordering van de expressie; - hFIXco sequentie: een codon geoptimaliseerde sequentie van de humane stollingsfactor IX; - SV40 poly A sequentie. De virale vector wordt geproduceerd door het bedrijf uniQure biopharma B.V. door middel van een baculovirusvectorexpressiesysteem, bestaande uit drie verschillende recombinante baculovirussen. Eén baculovirus codeert voor de AAV-2 Rep eiwitten, één voor AAV-5 Cap eiwitten en één levert het recombinante AAV-2 genoom met de hFIX expressiecassette. Insectencellen worden gecoïnfecteerd met deze baculovirusvectoren waarna productie van het ggo kan plaatsvinden. Vervolgens wordt het ggo gezuiverd. De klinische studie met AAV5-hFIX zal plaatsvinden in het Erasmus Medisch Centrum Rotterdam. Er worden in totaal 15 patiënten behandeld die een eenmalige dosis van 5x1012 genome copies per kilogram (gc/kg) of 2x1013 gc/kg toegediend krijgen. Deze doseringen zijn volgens de aanvrager equivalent aan ongeveer 5x1013 total particles per kilogram (tp/kg) en 2x1014 tp/kg. De vector wordt intraveneus toegediend met behulp van een programmeerbare infusiepomp.

Shire Sends Hemophilia Gene Editing Prospects Back to Sangamo There is no single race in gene therapy, or gene editing for that matter, more competitive than hemophilia. And if one of its entrants, Sangamo Biosciences, is going to come out on top, it’ll likely have to do it without the help of Shire. Sangamo Biosciences (NASDAQ: SGMO), a gene editing company based in Richmond, CA, said today that it’s restructured a wide-ranging alliance with Dublin-based Shire (NASDAQ: SHPG). Specifically, Shire has returned rights to Sangamo’s experimental programs for hemophilia A and B to the California company, and instead kept ahold of prospective treatments for Huntington’s disease and another unspecified program. All rights to other targets that were part of the original deal in 2012 have also been handed back to Sangamo, the two companies said. Sangamo didn’t disclose any specific financial details in the deal. Each company is paying the costs for its own programs, and each will get royalties on sales for the therapies it doesn’t own full rights to, up to a “maximum cap.” Shire still has a right of first negotiation to re-acquire the hemophilia programs in the future. Sangamo said that the two companies made this decision to focus on their respective areas of expertise. For Shire, that’s rare diseases like Huntington’s. Indeed, Sangamo said that the change will enable it to “accelerate the development” of its hemophilia A and B programs. Sangamo said that it’ll file papers by the end of the year to begin the first trial for its hemophilia B treatment. Still, as I wrote earlier this year, a super competitive race has taken shape to try to develop long-lasting—if not curative—treatments for hemophilia using gene therapy. Hemophilia was one of the first diseases tested with gene therapy, a method of delivering a healthy gene into a cell to replace a faulty or missing one. Yet nonetheless it’s taken a few decades and counting to make it work. Problems with the viral “vectors” used to deliver new genes into the body, and the immune response against those vectors, have proved a tall order. This recent editorial in Nature, for instance, details the balancing act companies are wrestling with in developing a safe, long-lasting treatment that doesn’t get shut down by the immune system. Even now, despite all of the scientific progress that’s been made over the years, only a few treatments are in early clinical testing. Yet the allure of a potentially curative treatment for hemophilia—currently managed by frequent, expensive injections of drugs that help clot blood—has attracted no fewer than seven groups trying to use either gene therapy, or, in Sangamo’s case, gene editing. The entrants: Dimension Therapeutics/Bayer, Spark Therapeutics/Pfizer, Biogen (NASDAQ: BIIB), Baxalta, UniQure (NASDAQ: QURE), Sangamo, and BioMarin Pharmaceutical (NASDAQ: BMRN). The differences between all of the gene therapy players is very technical. Some are using different types of adeno-associated virus and lentivirus vectors, which may elicit different types of immune responses and each have their purported strengths and weaknesses. Others are delivering different types of genes with these vectors. Baxalta and Spark, for instance, are using mutant therapeutic genes meant to clot blood much faster than normal—yet may produce unpredictable results. Recent actions taken by Spark management have exacerbated investor concerns. Uniqure (QURE - Get Report) will also be announcing some study results of its own this fall, but the data are early and preliminary, so not as pivotal to the future of the company. Both Spark and Uniqure are developing gene therapies for serious diseases. The recent clinical setbacks at Avalanche Biotechnologies and Celadon -- also gene therapy stocks -- put a chill in the market's love-a-thon with the emerging technology. It's a mistake to lump all gene therapy stocks together, but it would also be a confidence boost for the market to see positive results from a pivotal gene therapy study. Spark is next in line. Spark's lead gene therapy SPK-RPE65 is designed to improve vision in patients with inherited retinal dystrophies (IRD) -- a group of eye diseases affecting the retina. Patients with IRDs have decreased sensitivity to light that eventually leads to blindness. IRDs are caused by a mutation in a single gene. Spark's SPK-RPE65 is injected into the eye to correct a mutation to the RPE65 gene, the causative factor for about 3,500 IRD patients in the U.S. and Europe. FDA granted SPK-RPE65 with a Breakthrough Therapy Designation based on data from earlier-stage studies. As I mentioned above, Spark is running a randomized, placebo-controlled phase III study of SPK-RPE65 which is fully enrolled and expected to have top-line results before the end of the year. You can trace the weakness in Spark shares to management comments made Aug. 5. On a conference call discussing second-quarter earnings and a pipeline update, Spark CEO Jeff Marrazzo said this: On SPK-RPE65 we are reiterating our guidance to expect database lock and top-line results of our Phase III trial this year and BLA submission next year. We have reached the last patient/last visit in our study and are in the process of collecting all trial data. As we look ahead and fine-tune our preparations for BLA submission, we have taken advantage of our breakthrough therapy designation to engage in dialogue with FDA on certain details before we lock the database. We have continued to explore with FDA ways to enrich our analysis of the data collected in our Phase III trial. Given that these conversations are ongoing, we will not be discussing specific details today except to confirm our earlier guidance regarding timing of the data and BLA submission. We plan to provide an update before we release the Phase III results. [Emphasis added.] Investors freaked out about Marrazzo's disclosure of "dialogue with FDA" coming so close to the top-line results from the phase III study. Why is there a need for Spark to "enrich our analysis" of the SPK-RPE65 data? Absent a clear answer, investors sold off Spark. It's worth noting, too, that Spark only disclosed the new FDA chats on its Aug. 5 conference call but omitted any mention in the company's accompanying press release. Back to Uniqure: Preliminary data from a phase I/II study of its hemophilia B gene therapy are expected later this year, perhaps at the American Society of Hematology annual meeting in December. twitter.com/Prof_Dollar/status/639414... ik begreep eerder (ff kwijt of dat nu bij een presentatie van QURE of ergens anders was), dat QURE in trials weliswaar achter lijkt te lopen, maar qua voorbereidend werk ONCE weer goed zou kunnen inlopen. Dat had enerzijds te maken met de productiefaciliteit (opschaalbaarheid) en anderzijds een gedegener preklinisch traject, dat latere stadia zou kunnen versnellen. Door pre-klinisch goed huiswerk/voorbereiding te verrichten voorkom je onnodige kosten in latere stadia zoals fase I, II en III.
