Nog even belangrijke bijdrage plaatsen van avantiavanti 7-8-2015 inzake taaislijmziekte markt en concurrentie. Vertex Pharmaceuticals Inc. VRTX: Price: $130.85; Market Cap (MM): $31,936 Rating: Neutral; Price Target: $85.00 Andrew S. Fein The Start of Truly Personalized CF Medicine? Galapagos may be slowly becoming what Vertex professes to be. In our view, Vertex notched the first win in personalized CF medicine with the approval of Kalydeco in G551D. Last week, we believe we may have finally seen the next meaningful step towards truly personalized CF medicine, with applicability across the spectrum of genotypes: Galapagos (GLPG; not rated) licensed organoid technology from the HUB Foundation (for use in CF and IBD). We have previously highlighted this emerging technology and its potential in our industry reports: the in-vitro reproduction of human tissue with genetic and phenotypic disease characteristics creates not only more reliable testing models for novel CFTR modulators, but also serves as an ex-vivo diagnostic of drug response, thus personalizing treatment for each patient rather than for each genotype (see case of Kalydeco below). We applaud Galapagos for this move: it is precisely the type of investment in innovation that, in our view, Vertex should be making in order to stay competitive in a crowding CFTR field. Reinforcing the contrast between the two companies further, at the last ECFS meeting in Belgium, Galapagos and partner AbbVie (ABBV; not rated) also unveiled new in-house assays for the evaluation of CFTR correctors, focused on channel stability at the membrane, endocytosis and degradation. Meanwhile, in our view, Vertex compounds remain vulnerable on the channel stability count (both 809 and 661), which may or may not be an outcome of the preclinical models employed. Last but not least, beyond the preclinical, we also note that Galapagos and AbbVie might just beat Vertex to the first in-house triple combo for F508del. The Galapagos potentiator GLPG1837 is currently in Phase 1 and should move to Phase 2 before end-2015. Their corrector GLPG2222 should enter Phase 1 before end-2015, while their second corrector (third drug in their triple combo) should enter Phase 1 in 2Q16. We continue to believe that the Galapagos compounds have an edge on Kalydeco and VX-809 when it comes to preclinical potency. With these ongoing investments in precision/personalizing technology, in our view, the Galapagos pipeline is poised to quickly gain an edge both clinically and commercially. Who is a Kalydeco responder? Ask their organoids. The Dutch are doing it. Our most interesting takeaway from the recent ECFS meeting were reports of pilot real-time application of organoid technology to personalize CF treatment. Investigators from UMC Utrecht (where the HUB technology now licensed to Galapagos originated) profiled drug response in organoids from two patients with G1249R/F508del, which to date remains an uncharacterized genotype with no functional data published. After in vitro organoid work showed that this genotype was responsive to Kalydeco at levels typical in gating mutations (while at the same time showing little response to VX-809), the university clinical team placed both patients on Kalydeco therapy. The first patient began treatment with a baseline ppFEV1 of 30% (very ill; airway too damaged to expect a quick bounce in FEV1), but nonetheless responded to Kalydeco with improvement in NPD, sweat chloride, and airway resistance. The second patient began treatment with a baseline ppFEV1 of 50% (still lower than the typical mean baseline FEV1 of ~60% in many studies) and improved to 64% after only 4 weeks of treatment. Beyond the Kalydeco pilot, the Utrecht team believes that it may be able to leverage organoids to differentiate high and low responders to VX-809/Orkambi (only 4 weeks needed to culture patient organoids and get an answer on personalized drug profile). Importantly, the Utrecht team also notes that VX-661 and VX-809 appear relatively equally potent in their organoid models. This is in line with preclinical results from UNC Chapel Hill and McGill published last year, and lends further support to our thesis that VX-661 is unlikely to provide a pipeline hedge for VX-809 whether in a dual or triple combo, as more potent competitors mature. The implications of this organoid news, especially when viewed in the context of Galapagos’ recent in-licensing, are meaningful. In the near and medium term, organoids could allow Galapagos to identify and recruit ideal patients into clinical trials, bypassing the Vertex commercial pool (or by methodically poaching from it). Meanwhile, in the long term, the smart application of this technology could slice apart the Orkambi market in favor of newer and more potent agents, such as those currently in the clinic from Galapagos. Naturally, we identify Orkambi as a more likely target (rather than Kalydeco), given Orkambi’s mediocre FEV1 benefit across a population (some patients respond well, others not at all), and the absence of a test predictive of response in individual F508del patients. Such test, as well as alternative and more potent treatment options, may now be on their way, all courtesy of Galapagos. Valuation and risks. Our Vertex price target of $85/share is based on an equally-weighted composite of: (a) $95/share, as a 35x multiple of taxed and diluted FY20 GAAP EPS of $7.06 discounted back to FY15 at 20%; and (b) an NPV of $75/share (discount rate 10.5%, terminal growth rate of 2.5%). Risks to our investment thesis and target price include: (1) the failure of Kalydeco, Orkambi, VX-661 in further clinical studies; (2) the failure of Kalydeco and Orkambi to achieve our peak sales revenue; and (3) product competition.

"Kalydeco is een oraal toegediende CFTR potentiator voor de behandeling van patiënten van twee jaar en ouder met taaislijmziekte veroorzaakt door de Klasse III (G551D ) “gating” mutatie in het CFTR-gen" "Kleine molecule corrector benaderingen zijn gericht op het verbeteren van dysfunctioneel Klasse II CFTReiwit transport naar het celmembraan. Andere bedrijven die momenteel ook actief kleine moleculen correctoren aan het ontwikkelen zijn, zijn Vertex, Pfizer, Flatley Laboratories, Genzyme, Targeted Genetics en Bayer. Tot dusver zijn er geen goedgekeurde corrector moleculen op de markt". "Lumacaftor (VX-809), dat wordt ontwikkeld door Vertex, is een kleine molecuul corrector die in combinatie met Kalydeco voor activiteit getest wordt in patiënten met twee exemplaren (homozygoot) van de Klasse II (F508del) mutatie in het CFTR-gen". Deze 50% is een verwachte drempelwaarde die waarschijnlijk moet worden bereikt om volledige remissie van de ziekte te bereiken".
Vervolg: Galapagos’ taaislijmziekte moleculen zouden het potentieel kunnen hebben om belangrijk voordelen te bieden ten opzichte van de huidige goedgekeurde en in ontwikkeling zijnde therapieën:- ziekte modificerende activiteit op Klasse II/III mutaties- herstel van meer dan 50% van de CFTR activiteit, belangrijk voor het bereiken van overtuigende klinische werkzaamheid- betere risico / baten in vergelijking met huidige standaard van zorg- kleine moleculen voor orale toediening- voldoende veiligheid voor chronisch gebruik, waaronder pediatrische toepassing- geen nadelige interacties met andere geneesmiddelen vaak gebruikt door taaislijmziekte patiënten, waaronder antibiotica en ontstekingsremmers- werkzaam in homo- en heterozygote patiënten Galapagos kan zich mogelijk goed positioneren in taaislijmziekte wegens de volgende aspecten:- breed portfolio aan taaislijmziekte modulatoren, waaronder chemisch diverse molecuulseries met meervoudige binding modi om CFTR te moduleren- uniekeassay cascade, met inbegrip van primaire cellen van patiënten, voor descreening van kandidaatgeneesmiddelen die het CFTR-eiwit moduleren- ervaring sinds 2008 met het werken met uitgebreid discovery platform waarin zeer relevante ziekte assays op basis van cellen van patiënten met taaislijmziekte vervat zijn- samenwerkingsverband met AbbVie, die een expert is in combinatietherapieën en toegewijd aan het gebied van taaislijmziekte.

GLPG1837 is currently in a Phase 1 clinical trial with topline results expected in the third quarter of 2015. Our first oral corrector candidate, GLPG2222, is anticipated to start a Phase 1 trial in the second half of 2015. We anticipate nomination of a second corrector candidate in summer of 2015, such that we may have all three components of our triple combination therapy in development by mid-2015.

Univ. of Alabama at Birmingham, Birmingham, AL, USA; 2. Technion-Israel Institute of Technology, Haifa, Israel; 3. Galapagos NV, Generaal De Wittelaan L11A3, 2800 Mechelen, Belgium Premature termination codons (PTCs) in CFTR result in nonfunctional CFTR protein and are the proximate cause of ~11% of CF causing alleles. Previous studies have shown that the readthrough (RT) of PTCs and CFTR protein expression and function was restored by use of aminoglycosides (AGs; gentamicin, G418) and synthetic AGs (NB124). The CFTR potentiator ivacaftor (VX-770) was successfully used in restoring CFTR function in G551D and other related mutations. We have also shown that ivacaftor can augment CFTR function following RT by potentiating rescued CFTR channels. However, ivacaftor may have deleterious effects on CFTR stability, thus the evaluation of alternative potentiators is warranted. In this study, we tested if novel CFTR potentiators alone and in combination with translational RT could improve CFTR activity in comparison to ivacaftor. Stably transduced Fischer Rat Thyroid (FRT) G542X or R1162X epithelial cells when fully confluent, were pretreated with G418, NB124 (250?µg/mL) and transepithelial conductance (Gt) was recorded before and after CFTR stimulation by addition of forskolin (FSK, 100 nM or 10?µM) followed by investigational potentiators (GP-1, GP-2, GP-4; 10?µM) and VX-770 (10?µM). Similar studies were also carried out in primary HBE (human bronchial epithelial, F508del/G542X) cells for short circuit current (Isc) analysis. Furthermore, potentiators were also tested on FRT CFTR missense mutations with amino acid (AA) substitutions at the 542 position (Arg, Trp, Cys) that were created to study the functional consequences of AAs inserted during PTC suppression. GP-2 (0.67?±?0.07 Gt) and VX-770 (0.42?±?0.05) showed CFTR potentiator activity in wild-type (WT) CFTR, with GP-2 exceeding the effect of VX-770 by 63%. Different doses of FSK pre-stimulation (100 nM, 10?µM) did not significantly affect potentiator activity. In FRT G542X cells, ivacaftor (0.1?± 0.003), GP-2 (0.15?±?0.003), and GP-4 (0.1?± 0.005) augmented CFTR function compared to vehicle control (P<0.05), but only when RT was induced by NB124 or G418 pretreatment (P<0.01). In both FRT G542X and R1162X, GP-2 CFTR activity was significantly greater than VX-770 (P<0.05). Potentiators with or without RT agents had no effect on FRT cells without CFTR, confirming specificity. In FRT G542 AA mutants, GP-2 was active and directly proportional to the degree of CFTR function with FSK stimulation alone and also more efficacious compared to VX-770. In primary HBE G542X cells, GP-2 induced Isc (3.7?± 0.6?µA/cm2) was significantly greater in NB124 pre-treated cells compared to VX-770 (2.3?± 0.5?µA/cm2; P<0.05); total stimulated current (GP-2?+?FSK) approached 22.8% of WT CFTR. In conclusion these results indicate that the novel potentiator GP-2 is highly efficacious towards enhancing CFTR function following translational RT of PTCs and is superior to ivacaftor in this regard, even during chronic administration. Combination therapy (potentiators?+?RT agent) may be a useful approach to augment nonsense mutation CFTR function rescued by translational RT.

Ja, maar ik heb er alleen nr. 219 uitgekozen en die dan volledig gepost. Deze poster geeft nl. zeer interessante informatie. Zo zien we nog maar eens dan in preklinische proeven GLPG 1837 veel beter werkt dan Kalydeco. Bovendien zijn deze testen uitgevoerd op een klasse van mutaties die door Vertex niet behandeld wordt. Een zeer belangrijke klasse overigens want het gaat over zo'n 10% van het totaal aantal CF patiënten.

Heeft het CF-programma kans om een een fast-track status te krijgen?
Abbvie heeft GLPG1837 in licentie. Hoort hier ook bij de 3e corrector bij die de werking tegen CF (taaislijm) zoals het er naar uit gaat zien met 20% gaat verbeteren van 40% naar 60%?

Ja, valt onder de licentie. De gehele triple combinatie valt daaronder. Nog 350 Miljoen aan mijlpaalbetalingen in verschiet voor Galapagos. Daar zal Galapagos in Q4 2015 een deel van ontvangen. -voltooien Fase1 potentiator en start fase 2, opstart Fase1 1e corrector en selectie 2e corrector
