....... Eine weitere – allerdings weniger erfreuliche – „Premiere“ kann aus der Sitzung des G- BA vom 16. April 2015 berichtet werden. Erstmals seit der Einführung des AMNOG hat der G-BA beschlossen, die gesetzliche Frist für die Durchführung der Verfahren zur frü- hen Nutzenbewertung nicht einzuhalten, indem er die Beschlussfassung vertagt hat. Dieser Vorgang ereignete sich im Rahmen des Nutzenbewertungsverfahrens zum Wirk- stoff Alipogentiparvovec, Handelsname Glybera®, einem Orphan Drug zur Behandlung der sehr seltenen Stoffwechselerkrankung Lipoproteinlipasedefizienz (LPLD). Dieser Wirkstoff ist ein sogenanntes Orphan Drug zur Behandlung seltener Erkrankungen, für die bereits mit der Zulassung über das Mittel einer gesetzlichen Fiktion der Zusatznut- zen als belegt gilt. Eine Regelung, die unter anderem dazu dient, dass Arzneimittel in solchen nur mit geringer Patientendichte versehenen Populationen eine Privilegierung erfahren und dass erhöhte Forschungsanreize in diesem durch geringe Absatzzahlen gekennzeichneten Bereich gesetzt werden. Für Glybera® hätte auf dieser Grundlage in der Plenumsentscheidung vom 16. April 2015 der entsprechende Nutzenbewertungsbeschluss gefasst werden müssen, was seitens des G-BA allerdings nicht erfolgt ist. Hintergrund der ungewöhnlichen Ausset- zungsentscheidung ist der dem G-BA erst am 14. April 2015 bekannt gewordene Um- stand, wonach der zuständige Rapporteur der europäischen Zulassungsbehörde EMA in einem Bericht vom 8. April 2015 mitgeteilt hat, dass er nach Auswertung der Follow-Up- Daten 2014 ein negatives Nutzen-Risiko-Verhältnis für den Wirkstoff sieht. Das zustän- dige Beschlussgremium der EMA (CHMP) befasst sich in seiner Sitzung vom 21. –23. April 2015 mit möglichen Folgerungen aus dem Rapporteursbericht. Auch wenn wir mit der Aussetzung des Verfahrens die gesetzliche Verfahrensfrist ver- letzen, ist die Entscheidung aus meiner Sicht richtig und alternativlos. Der Schutz der Sicherheit von schwerkranken Patientinnen und Patienten hat absoluten Vorrang. Ausgehend von den Mitteilungen über den Rapporteursbericht müssen dessen Feststellun- gen gravierend sein. Auch wenn dieser Bericht nur das Bewertungsergebnis des Rap- porteurs darstellt, wäre es unverantwortlich, dem Wirkstoff aufgrund der Privilegierung für Orphan Drugs vor der anstehenden Empfehlung des CHMP einen Zusatznutzen aus- zusprechen und damit ein möglicherweise falsches Signal an Patienten und Verordner zu senden, die sich an den Beschlüssen des G-BA orientieren. Die Sicherheit der Patien- ten ist wichtiger als die Einhaltung einer formalen gesetzlichen Verfahrensfrist, wenn ein solch gravierender atypischer Sonderfall vorliegt. Dass der G-BA ansonsten die Fris- ten peinlichst genau beachtet, ist in den circa 120 bislang durchgeführten Verfahren bewiesen worden. In diesem Kontext befremdlich war für mich die Tatsache, dass der G-BA über die Um- stände erst einige Tage nach der US-Börsenaufsichtsbehörde SEC unterrichtet wurde. Bereits in der 15. Kalenderwoche hatte der amerikanische Zulassungsinhaber in einer ad-hoc-Meldung die Börsenaufsicht mit Blick auf mögliche Geschäftsrisiken unterrich- tet, erst am 14. April erfolgte eine Meldung durch den pharmazeutischen Unternehmer an den G-BA. Ich hätte erwartet, dass angesichts des Wissens um das beim G-BA lau- fende Bewertungsverfahren zumindest eine zeitgleiche Unterrichtung von Börsenauf- sicht und G-BA erfolgt, denn der Patientenschutz ist für mich in solchen Fällen wichtiger als mögliche geschäftspolitische Imponderabilien. ........... www.g-ba.de/downloads/33-211-140/02-2...

Zonder verdure achtergronden te kennen: het lijkt alsof het vandaag op de agenda stond om Glybera uit het proces te halen als de CHMP beslissing negatief was geweest. Een handhaving door de CHMP leidt kennelijk niet automatisch tot een beslissing om Glybera op te nemen. Snap ik toch niet helemaal. Wellicht probeert de GBA een beslissing nu uit te stellen tot de CHMP de antwoorden van uniQure op de nadere informatie die gevraagd is heft beoordeeld en de registratie definitief handhaaft. Iemand hier inzicht in? Andere lijkt me dit een vraag voor AAV.

Glybera Regulatory Status On April 8, 2015, the Company received a copy of a preliminary assessment report on Glybera prepared by the rapporteur designated by the Committee for Advanced Therapies (CAT), which is the committee that advises the Committee for Human Medicinal Products (CHMP) on gene therapies. The preliminary report was a response to the Company’s submission to the European Medicines Agency (EMA) on September 5, 2014 of a Type II variation, which proposed an amendment to the Glybera Summary of Product Characteristics (SPC) to reflect certain information from the six year follow up data included in the Company’s final clinical study report. The preliminary assessment report, which represented the sole view of the rapporteur, stated that Glybera lacked efficacy and therefore the benefit-risk balance was negative. The rapporteur’s preliminary report was provided to the CAT for further discussion in advance of the CAT’s monthly meeting on April 16-17. On April 24, the Company received a copy of the final assessment report prepared by the CAT and endorsed by the CHMP, which states the following: “At the April CAT meeting, the CAT discussed the negative rapporteur recommendation on the benefit risk of Glybera. The CAT did not agree with the negative view of the rapporteur and concluded by majority on the following recommendation presented below: “The efficacy of Glybera needs to be considered in its totality as defined in the initial approval taking into account, the criteria considered at time of initial approval: · the persistence of LPL (lipoprotein lipase) activity · the evidence of an effect on lipids, in particular the post prandial CM (chylomicron), the evidence presented on the reduction in the rate of pancreatitis” In accordance with the Company’s Type II variation request, the CAT will continue to evaluate the six year follow up data and has requested supplemental information, which the Company is currently preparing. On April 28, the Company informed the Federal Joint Committee (G-BA), which is responsible for the commercialization of Glybera in Germany, of its receipt of the final assessment report from the CAT. Previously, the G-BA had put its ongoing benefit assessment of Glybera on hold to await the final assessment of the CAT and the CHMP regarding benefit/risk. Based on the recommendations stated in the final assessment report, the Company has requested the G-BA to immediately resume its benefit assessment of Glybera The Company continues to believe that the clinical data from its Glybera development program, including the six-year follow-up data, support the long-term value and efficacy. However, the Company can provide no assurance regarding the final conclusions of the EMA and G-BA. Is it worth $1 million to treat a potentially fatal genetic disease that causes painful swelling of the pancreas and can require repeated trips to the emergency room? Germany will be first to answer that question. Health authorities there will rule Thursday on whether state-funded insurers should pay that price for the drug, the first gene therapy approved for use in Europe. It will be a milestone for the drugmaker, UniQure NV of Amsterdam, and for other expensive medicines that target genetic defects. “Glybera, a pioneer product in gene therapy, is targeting an exceptionally small market,” said Gbola Amusa, an analyst at Chardan Capital Markets LLC in New York who recommends buying UniQure shares. The cost “is likely to be far above the price of future gene therapies.” Glybera’s price tag reflects the fact that it’s given just once and targets a disease that afflicts about 200 people in Europe. Gene therapies aim to fix or knock out defective genes, or add those that are missing, to cure diseases. Glybera’s use has been limited to patients in trials, and UniQure hasn’t yet filed it to U.S. regulators. The cost should be compared with other orphan drugs that target rare diseases, which on average cost about $250,000 a year, UniQure Chief Executive Officer Jorn Aldag said in a phone interview. A study of 19 patients showed that Glybera was effective over six years, making the $1 million price equivalent to less than $170,000 a year. In addition, the therapy can limit hospital stays and long-term complications such as heart disease and diabetes. “It is often hard to be the first,” Aldag said. “This is where people are learning. Not only are we learning, but so are the regulatory agencies.” Hot Field After years of setbacks over safety, gene therapy is now one of the hottest areas in biotechnology. Spark Therapeutics Inc., a Philadelphia-based company that is developing a gene therapy for restoring sight, hit a $1.2 billion valuation on its first day on the market in January. Shares of Bluebird Bio Inc., which is working on treating a rare blood disorder, have risen 10-fold since a June 2013 IPO. UniQure shares have almost tripled over 12 months, valuing the company at about $585 million. Bristol-Myers Squibb Co. agreed last month to work with the company and buy a stake of as much as 20 percent. Glybera treats lipoprotein lipase deficiency, where an enzyme is lacking to process fat in the intestine, resulting in acute pancreatic inflammation. To fix the deficiency, an engineered copy of the LPL gene is injected into leg muscle. Final Price The final price in Germany will be negotiated with health insurance funds, which will use Thursday’s assessment as a guide, according to Chiesi Farmaceutici SpA, the marketing partner for Glybera in Europe. For other gene therapies, buyers won’t necessarily have to pay the full price upfront. Payments may be spread out over several years and could cease if a patient dies, Aldag said. European regulators approved Glybera in 2012, after denying it three times. They required UniQure to provide six years of follow-up data, which was supplied in June. The study results show that bouts of pancreatitis and hospitalization rates were reduced by about half. Still, the European Medicines Agency is reviewing whether the treatment has a lasting effect. That may become an issue for Spark Therapeutics. A gene therapy similar to the one it’s developing to restore sight in patients with a rare form of childhood blindness may wane with time, two studies showed this month. The studies didn’t use the same technique or active therapy as the trials underway from Spark. While Glybera is an important milestone for gene therapy, a treatment being tested for hemophilia may hold greater value for UniQure investors, Chardan’s Amusa said. Early-stage trial results for the product, AMT-060, will be ready in the second half of the year. Bijkomende voordelen van Glybera® (werkzame stof: Alipogentiparvovec) kwantificeerbare Berlijn, 21 mei 2015 - Het Gemengd Comité Federal (G-BA) heeft donderdag in Berlijn, de omvang van de wet veronderstelde meerwaarde van Glybera®, een geneesmiddel voor een zeldzame metabole ziekte te behandelen (Lipoproteinlipasedefizienz - LPLD), geclassificeerd als niet-kwantificeerbaar. De door de farmaceutische fabrikanten ingediende wetenschappelijke gegevens werden naar aanleiding van de evaluatie van de G-BA geen betrouwbare technische informatie over de omvang van de extra voordeel. Voor geneesmiddelen die worden gebruikt om een ??zeldzame ziekte (orphan drugs) te behandelen, het bijkomende voordeel is geldig tot een verkoop limiet van € 50.000.000 als gevolg van een wettelijke bepaling als bezet. Zolang de drug legaal een vergunning is dus - onafhankelijk van de gegevens en kennis om de positie van G-BA aanwezig is - een extra voordeel voor Glybera® te insinueren. De G-BA kan de uitkomst van de batenanalyse besluiten alleen op de omvang van de extra voordeel. Aangezien de kans op verdere gegevens latere eisen van Wm over de effectiviteit en veiligheid van Glybera®, heeft de G-BA de geldigheid van de beschikking afloopt op 1 juni 2016. Op dit moment wordt een nieuwe uitkering evaluatie gestart.
Heb het eea in het duitse origineel nagelezen en gezien dat Glybera tot 1 juni 2016 volledig wordt vergoed in Duitsland! Tijdens behandeling van patienten moet QURE van de EMA gegevens verzamelen en deze zullen dan door de GBA worden gebruikt om een nieuwe nutzenbewertung te starten. Kortom tijd genoeg om alle Duitse patienten te behandelen lijkt me.... Een klinkend success, met vooral psychologische impact, want meer dan 50 miljoen gaat er niet worden omgezet......

als ik als leek het zo vrij mag vertalen: werkzaamheid lastig uit te drukken en verder geen discussie over de veiligheid. Voorlopig niets aan de hand dus...vergoeding wordt gegeven, zij het tot 50 mln en na de goedkeuring vd EMA zijn er geen nieuwe patienten behandeld, waardoor de GBA amper anders kan oordelen dan vergoeden en per 1 juni 2016 opnieuw evalueren. (wordt het vanaf heden dringen van patienten in Duitsland om die behandeling met vergoeding te verkrijgen?) De uitspraak lijkt misschien negatief, maar de behandeling met Glybera kan gewoon plaatsvinden en zelfs worden vergoed. Ik denk ook dat QURE door deze uitspraak verder kan gaan met het opbouwen van kennis, ervaring en uitwerken van gentherapiën.
Helemaal eens. Dit betekent gewoon dat alle patienten die er nu zijn behandeld kunnen worden. En 1 injectiesessie is genoeg. Vergoeding 1,1 miljoen euro. Herziening beslissing per 1 juni 2016 als er nieuwe data zijn tenminste. EMA gaat over registratie, GBA gaat alleen over toegevoegde waarde en als dat oordeel gegeven is (zoals bij Glybera) ben je gewoon verged voor de VOLLE 1,1 miljoen euro per patient.
