GLPG2665 is the first candidate to complete the potential triple combination therapy for the delta F508 (class II) mutation in cystic fibrosis. GLPG2665 will now enter pre-clinical development, and is expected to enter Phase 1 studies by mid 2016. GLPG2665 in combination with corrector GLPG2222 and potentiator GLPG1837 consistently have shown restoration of healthy activity level in human bronchial epithelial (HBE) cells of patients with the Class II F508del mutation. The combination resulted in chloride transport up to six-fold greater than Orkambi in HBE cells with the homozygous F508del mutation. Poor functioning of the CFTR channel is the major deficit in patients with cystic fibrosis. About the Galapagos - AbbVie collaboration in cystic fibrosis In September 2013 Galapagos signed an agreement with AbbVie in which they will work collaboratively to develop and commercialize oral drugs that address the main mutations in CF patients, including F508del and G551D. Under the terms of the agreement, AbbVie made an upfront payment of $45 million to Galapagos. Upon successful completion by Galapagos of clinical development through to completion of Phase II, AbbVie will be responsible for Phase III, with financial contribution by Galapagos. Galapagos achieved a $10 million milestone in 2014 and is eligible to receive up to $350 million in total additional payments for developmental and regulatory milestones, sales milestones upon the achievement of minimum annual net sales thresholds and additional royalty payments on net sales, ranging from mid-teens to 20%. About cystic fibrosis (CF) CF is a rare, life-threatening, genetic disease that affects approximately 80,000 patients worldwide and approximately 30,000 patients in the United States. CF is a chronic disease that affects the lungs and digestive system. CF patients, with significantly impaired quality of life, have an average lifespan approximately 50% shorter than the population average, with the median age of death at 27. There currently is no cure for CF. CF patients require lifelong treatment with multiple daily medications, frequent hospitalizations and ultimately lung transplant, which is life-extending but not curative. CF is caused by a mutation in the gene for the CFTR protein, which results in abnormal transport of chloride across cell membranes. Transport of chloride is required for effective hydration of epithelial surfaces in many organs of the body. Normal CFTR channel moves chloride ions to outside of the cell. Mutant CFTR channel does not move chloride ions, causing sticky mucous to build up on the outside of the cell. CFTR dysfunction results in dehydration of dependent epithelial surfaces, leading to damage of the affected tissues and subsequent disease, such as lung disease, malabsorption in the intestinal tract and pancreatic insufficiency.

Volgens mij is dit inderdaad fantastisch nieuws. Op de CF conference vorige week in Phoenix gaf Vertex de data over hun triple combi therapie. Zij komen tot een verdrievoudiging in chlloridetransport tov Orkambi. Galapagos spreekt nu over een verzesvoudiging!!!! Kun je nagaan war dit betekent.
Wat ik interessant vind (behalve 6x Cl transport natuurlijk) is dat er consequent gesproken wordt over 'series' en 'first candidate'. Zou dat betekenen dat er meerder combo's zullen onderzocht (en misschien ontwikkeld) worden, of is dit de degene met de grootste potentie en worden de anderen pas onderzocht als GLPG2665 niet het verwachte resultaat oplevert?

Ik denk dat jouw interpretatie juist is er zijn verschillende mutaties binnen CF dus is plausibel dat daar ook verschillende therapieen voor ontwikkeld worden

Fein H.C.WAINWRIGHT&CO The Start of Truly Personalized CF Medicine? Galapagos may be slowly becoming what Vertex professes to be. In our view, Vertex notched the first win in personalized CF medicine with the approval of Kalydeco in G551D. Last week, we believe we may have finally seen the next meaningful step towards truly personalized CF medicine, with applicability across the spectrum of genotypes: Galapagos (GLPG; not rated) licensed organoid technology from the HUB Foundation (for use in CF and IBD). We have previously highlighted this emerging technology and its potential in our industry reports: the in-vitro reproduction of human tissue with genetic and phenotypic disease characteristics creates not only more reliable testing models for novel CFTR modulators, but also serves as an ex-vivo diagnostic of drug response, thus personalizing treatment for each patient rather than for each genotype (see case of Kalydeco below). We applaud Galapagos for this move: it is precisely the type of investment in innovation that, in our view, Vertex should be making in order to stay competitive in a crowding CFTR field. Reinforcing the contrast between the two companies further, at the last ECFS meeting in Belgium, Galapagos and partner AbbVie (ABBV; not rated) also unveiled new in-house assays for the evaluation of CFTR correctors, focused on channel stability at the membrane, endocytosis and degradation. Meanwhile, in our view, Vertex compounds remain vulnerable on the channel stability count (both 809 and 661), which may or may not be an outcome of the preclinical models employed. Last but not least, beyond the preclinical, we also note that Galapagos and AbbVie might just beat Vertex to the first in-house triple combo for F508del. The Galapagos potentiator GLPG1837 is currently in Phase 1 and should move to Phase 2 before end-2015. Their corrector GLPG2222 should enter Phase 1 before end-2015, while their second corrector (third drug in their triple combo) should enter Phase 1 in 2Q16. We continue to believe that the Galapagos compounds have an edge on Kalydeco and VX-809 when it comes to preclinical potency. With these ongoing investments in precision/personalizing technology, in our view, the Galapagos pipeline is poised to quickly gain an edge both clinically and commercially. Who is a Kalydeco responder? Ask their organoids. The Dutch are doing it. Our most interesting takeaway from the recent ECFS meeting were reports of pilot real-time application of organoid technology to personalize CF treatment. Investigators from UMC Utrecht (where the HUB technology now licensed to Galapagos originated) profiled drug response in organoids from two patients with G1249R/F508del, which to date remains an uncharacterized genotype with no functional data published. After in vitro organoid work showed that this genotype was responsive to Kalydeco at levels typical in gating mutations (while at the same time showing little response to VX-809), the university clinical team placed both patients on Kalydeco therapy. The first patient began treatment with a baseline ppFEV1 of 30% (very ill; airway too damaged to expect a quick bounce in FEV1), but nonetheless responded to Kalydeco with improvement in NPD, sweat chloride, and airway resistance. The second patient began treatment with a baseline ppFEV1 of 50% (still lower than the typical mean baseline FEV1 of ~60% in many studies) and improved to 64% after only 4 weeks of treatment. Beyond the Kalydeco pilot, the Utrecht team believes that it may be able to leverage organoids to differentiate high and low responders to VX-809/Orkambi (only 4 weeks needed to culture patient organoids and get an answer on personalized drug profile). Importantly, the Utrecht team also notes that VX-661 and VX-809 appear relatively equally potent in their organoid models. This is in line with preclinical results from UNC Chapel Hill and McGill published last year, and lends further support to our thesis that VX-661 is unlikely to provide a pipeline hedge for VX-809 whether in a dual or triple combo, as more potent competitors mature. The implications of this organoid news, especially when viewed in the context of Galapagos’ recent in-licensing, are meaningful. In the near and medium term, organoids could allow Galapagos to identify and recruit ideal patients into clinical trials, bypassing the Vertex commercial pool (or by methodically poaching from it). Meanwhile, in the long term, the smart application of this technology could slice apart the Orkambi market in favor of newer and more potent agents, such as those currently in the clinic from Galapagos. Naturally, we identify Orkambi as a more likely target (rather than Kalydeco), given Orkambi’s mediocre FEV1 benefit across a population (some patients respond well, others not at all), and the absence of a test predictive of response in individual F508del patients. 15/10/15 Reactie Jos Versteeg op vordering CF (van IEX.nl) AMSTERDAM (AFN) - De door Galapagos aangekondigde voortgang in het onderzoek naar een middel tegen taaislijmziekte is belangrijk nieuws. Dat stelde analist Jos Versteeg van Theodoor Gilissen donderdag. Het biotechnologiebedrijf liet eerder op de dag weten dat de tweede corrector (genaamd GLPG2665) in het onderzoek naar de preklinische fase wordt gebracht. De onderneming verwacht Fase 1 in de eerste helft van 2016 te starten. Volgens Versteeg zijn Galapagos en samenwerkingspartner AbbVie nu dichter bij het doel om een medicijn te vinden voor patiënten met taaislijmziekte, dat daadwerkelijk de oorzaken van de ziekte aanpakt. Galapagos kan het succes bij de ontwikkeling van medicijnen tegen taaislijmziekte goed gebruiken bij de onderhandelingen over de ontwikkeling van filgotinib, stelde de analist verder. AbbVie gaf onlangs de licentie van dit medicijn terug en Galapagos is op zoek naar een nieuwe partner.

The plan is to advance the third component of a triple-combination therapy into the clinic next year, setting the partners up to trial a cocktail intended to hit multiple targets of relevance to cystic fibrosis. GLPG2665, a compound designed to prevent misfolding of the CFTR protein, is seen by Galapagos as the final piece of its multifront attack on cystic fibrosis. The compound is set to enter Phase I next year--before which preclinical testing must be completed--but in the longer term its fate is tied to that of two other assets already in development at Galapagos, GLPG2222 and GLPG1837. GLPG2222, another drug to correct protein misfolding, is advancing down the pipeline just ahead of GLPG2665. A filing for a trial in healthy volunteers is pencilled in for before the end of the year. The third component of the triple therapy, GLPG1837, is nearing the start of Phase II. GLPG1837 has a different role in the cocktail than GLPG2222 or GLPG2665. Galapagos sees GLPG1837 working to boost the activity of defective CFTR, potentially making it complementary to the misfolding-fixing function of the other two compounds. Filgotinib is currently being assessed as a treatment for Crohn’s disease (CD) in a Phase 2 study. It has been shown to be particularly efficacious in arthritis as well as colitis mouse models. Here, we report that in mouse and human colon after treatment with filgotinib in vivo or ex vivo, respectively, the same signaling mechanisms are impacted, suggesting common JAK1 inhibition-related effects. AIMS & METHODS: Chronic colitis was induced by adding 4% dextran sodium sulfate (DSS) to drinking water of Balb/c mice that were administered 30 mg/kg filgotinib QD during the 16 days of the study. Disease activity index (DAI) was evaluated daily by measuring weight loss, rectal bleeding, and stool consistency. Histological scoring of disease was based on an evaluation of severity and extent of inflammation and epithelial damage. Immune cell infiltration as well as STAT activation in mouse gut was analyzed by immunohistochemistry. Gene expression analysis in the colon mucosa was performed using qPCR. Inflamed colon biopsies from IBD patients were cultured for 18 or 24 hours in the presence of 5 µM GLPG0634 and gene expression as well as STAT phosphorylation were analyzed using qPCR and 5-Plex STAT kit Luminex, respectively. RESULTS: DAI revealed the strong protection against experimental colitis conferred by filgotinib. This was confirmed by histological assessment, which showed lower disease severity (54%), as well as reduced inflammatory cell infiltration in colon tissue from filgotinib-treated mice compared to controls. Colons from DSS-treated mice displayed higher STAT3 phosphorylation levels, which were prevented by treatment with filgotinib. Of interest, the expression of genes that activate (IL-6) or are induced by the JAK1 pathway (MX1, MX2) followed the same regulation as pSTAT3, suggesting a relationship between these readouts and a role of the pathway in the disease. Remarkably, the same repression of IL-6 and MX1 expression, as well as STAT3 phosphorylation by filgotinib were observed in cultured colon biopsies from IBD patients. CONCLUSION: Taken together, these new data provide further mechanistic understanding for the pronounced efficacy of JAK1 inhibition in the pre-clinical mouse DSS-induced colitis model, with the efficacy observed at the macroscopic, histologic and molecular levels correlating with the inhibition of STAT3 phosphorylation in the colon. The striking concordance between molecular signatures related to JAK1 inhibition in experimental colitis and in a human colon biopsy model further stresses the importance of the JAK1 pathway in the etiology of IBD.
