I have to agree with MadisonAV. I've been quite optimistic about the prospects for filgotinib (thought filgotinib could become a megablockbuster over the next few years). An important element was ofcourse the FDA approval for filgotinib. Then Gilead/Galapagos announced on Augustus 18 that the FDA had handed it a complete response letter (CRL) for filgotinib. The agency requested data from the Manta and Manta-Ray studies, neither of which will wrap up until next year. Gilead also said that the FDA "expressed concerns regarding the overall benefit/risk profile of the filgotinib 200 mg dose."The FDA decision delays a potential launch by 12-18 months, which weakens filgotinib's commercial position. If the 200 mg dose of filgotinib is eventually excluded from the label, the 100 mg dose is unlikely to be competitive in my opinion. I'm now concerned that Gilead's entire filgotinib program could be at risk. But the FDA rejection raises even more questions in my mind. Before Dan O'Day took the helm as Gilead's CEO, it seemed likely that Gilead would have to wait to file for FDA approval of filgotinib until the Manta study was completed, because of the agency's safety concerns about the drug. However, in O'Day's first quarterly conference call as CEO in May 2019, it was abundantly clear he was pushing hard to speed up the regulatory submission. In hindsight, it would have been more prudent to make sure that filgotinib wouldn't run into problems. Filgotinib was a large sum of the part valuation for the GLPG stock. That scenario is much less likely now.The upcoming catalysts are indeed very important for the company. Dit is een advies, vergelijkbaar met CHMP advies. De goedkeuring volgt dan weer een tweetal maanden later.
Abbvie heeft naar aanleiding van het positief advies geen persbericht uitgebracht, wel naar aanleiding van de goedkeuring

Op de website van Pharma Japan zal er wel een aankondiging zijn in geval van een positief advies.
Ik lees: A thumbs-up from the panel will put them in line for approval as early as September. M.i. zou dat dus betekenen dat ze sneller dan die 2 maanden goedkeuring zouden kunnen geven. Ook wel logisch, het inhoudelijke oordeel is dan al geveld.

Het risico is hard gestegen (op dit moment). Voorlopig is Galapagos nog geen farmaceut. Maar we hebben het hier wel over een biotech bedrijf met veel kansen en belachelijk veel geld.

@Wall Street Trader @Madison Av Ook ik ben enorm geschrokken van de voorlopige FDA afwijzing, en ook mijn vertrouwen in Galapagos heeft een flinke knauw gekregen. Maar volgens mij is er zowel in de beurskoers als in wat de meeste forum-leden schrijven sprake van een sterke overreactie. Is het redelijk om te geloven dat de Galapagos pipeline op dit moment zo'n 25-30 Euro waard is? Dus dat het bedrijf veel minder waard is dan pakweg in 2015? Nee toch? Zelfs als we afzien van filgotinib in RA is Galapagos als bedrijf toch veel verder dan 5 jaar geleden? Er wordt nu gedaan alsof het min of meer afgelopen is met filgotinib, maar dat is natuurlijk helemaal niet zo. De verwachting is nog steeds dat het in Europa en Japan dit jaar van start kan. De vertraging in de VS is een kolossale tegenvaller, maar het laatste woord is hier nog niet over gezegd. Zie ook de reactie van Rekyus in het 'Inhoudelijk Light' draadje: er zijn allerlei scenario's denkbaar, variërend van (in het slechtste geval) 1.5 jaar of meer vertraging tot afspraken met de FDA waarin het sneller loopt, eventueel met een eerdere 100mg dan 200mg toelating. Zonder twijfel worden de scenario's op dit moment doorgesproken; misschien horen we daar op 8 september al iets meer over. Filgotinib wordt naast RA getest in een hele serie andere indicaties, waarvan enkele ver gevorderd zijn, en het lijkt me op dit moment gewoon ondenkbaar dat Gilead de stekker uit filgotinib zou trekken na alle gedane investeringen en na één tegenslag. Merk op dat ondanks alle afwaarderingen van analisten de gemiddelde door hen toegekende waarde nu nog steeds rond de 130 Euro ligt. Zelfs als de waarde van filgotinib 0 zou zijn, wat niet zo is gezien Europa en Japan en de andere indicaties naast RA, is de huidige beurskoers nog erg bescheiden. Iemand noemde Galapagos op dit moment een gokaandeel, en in zekere zin zijn alle biotech aandelen dat omdat je nooit weet of er uiteindelijk een pilletje zal komen, maar juist Galapagos heeft zoveel pijlen op zijn boog dat het (als het al een gok is) wel een beredeneerde gok is: er zijn goede kansen in IPF, er zijn serieuze verwachtingen voor Toledo, wie weet levert OA iets op, er is het discovery platform, en er is nog het een en ander. Natuurlijk: al die dingen kunnen op niets uitlopen, niets is zeker. Maar dat was in 2015 nog veel meer het geval want alle programma's waren toen een stuk minder ver, en toen was de beurskoers verhoudingsgewijs al veel hoger dan nu. Wat nu gebeurt is in mijn ogen gewoon irrationeel. En als werkelijk alles hopeloos mocht blijken te zijn, wat ik helemaal niet verwacht, dan is er mogelijkerwijs altijd nog de noodrem: want het lijkt me buitengewoon aantrekkelijk voor Gilead om Galapagos te kopen. Niet alleen is dat financieel interessant vanwege de berg cash en de 25% of meer aandelen die ze al hebben, maar ook koopt men dan een enorme berg know-how in termen van lopende onderzoeken en specialistische kennis en mankracht. Natuurlijk zijn de komende weken en maanden cruciaal: wat wordt er gezegd op 8 september; komt er geen kink in de kabel in de EC en Japan; hoe komt het nieuwe tijdpad in de VS er uit te zien; hoe stelt Gilead zich op; zijn er mooie uitslagen bij minstens één en liefst twee of drie van de binnenkort bekend gemaakte PINTA, ROCCELLA en NOVESA uitslagen. Samengevat wat mij betreft: ik ben enorm geschrokken en heb (op papier) een zeer stevige financiële tik gehad, maar ik zie op dit moment geen reden om mijn vertrouwen in Galapagos op te zeggen en voel me daarin gesteund door de posts van inhoudelijk onderlegde forumleden als met name Rekyus en WiC32. Misschien ziet alles er over een maand alweer wat zonniger uit.

Jij hebt voorkennis :-) Ik vind de pijplijn van Arrrowhead, maar ook hun werkingsmechanisme, veel indrukwekkender. Kan nog van alles mis gaan, maar als er 3 van de huidige 13 goed gaan, dan stellen ze GLPG in de schaduw. Mijn positie in Arrowhead is dus groter, en was dat al voor 19 augustus.

Filgotinib (Jyseleca) has NOT been officially approved in Japan yet! Next step would be the Ministry (same as Europe) -> MHLW. Its been OK'd by the CHMP of Japan (to make comparison).

De bedrijven voelen zich in elk geval gesterkt door de positieve adviezen in Europa en Japan , waar de goedkeuring van het middel als behandeling tegen reuma daarmee een formaliteit lijkt. Filgotinib Effective, Safe for Psoriatic Arthritis September 11, 2020 Samara Rosenfeld Filgotinib has a similar safety profile for psoriatic arthritis as it does for rheumatoid arthritis, according to the findings of new research presented at the Congress of Clinical Rheumatology East 2020 meeting. No new or unexpected safety signals were discovered, while filgotinib was generally well-tolerated and sustained efficacy up to open-label extension at 52 weeks. Laura Coates, MBChB, MRCP, PhD, and investigators evaluated the interim safety and efficacy of filgotinib 200 mg in patients participating in EQUATOR2 at 52 weeks’ treatment. Phase 2 of the EQUATOR study, a 16-week, double-blind, randomized controlled trials demonstrated filgotinib significantly improved multiple disease domains versus placebo and was well-tolerated among patients with psoriatic arthritis. EQUATOR2 is ongoing and assessing the long-term safety and efficacy of the therapy for up to an additional 148 weeks of treatment. Patients with active psoriatic arthritis were randomized 1:1 to receive either oral filgotinib 200 mg or placebo once daily for 16 weeks in EQUATOR. Those included were at least 18 years old with psoriatic arthritis defined by Classification for Psoriatic Arthritis criteria, were diagnosed at least 12 weeks before screening, had moderate-to-severe psoriatic arthritis (at least 5 swollen joints and at least 5 tender joints), active or history of plaque psoriasis, and insufficient response to at least 1 conventional synthetic disease-modifying antirheumatic drug (csDMARD). Those who completed EQUATOR were eligible for EQUATOR2. In EQUATOR2, patients received open-label oral filgotinib 200 mg once daily for up to 148 weeks. Of those enrolled in EQUATOR, 87% enrolled in the open-label extension. Overall, half of patients were female, the mean age was 50 years old, and the mean duration of psoriatic arthritis was 7 years. Some patients (15%) received prior anti-tumor necrosis factor therapy and 74% received concurrent csDMARD therapy at baseline and during the study. In the filgotinib and placebo groups, median Psoriasis Area and Severity Index score was 6.5 (IQR, 2.6-15) and 6.9 (IQR, 3.8-18.6), respectively. The mean Psoriatic Disease Activity score was 6.1 for filgotinib and 6.2 for placebo, and the mean Disease Activity Index for psoriatic arthritis score was 44 and 47.8 for filgotinib and placebo, respectively. Total filgotinib exposure was 160 patient-years with a median time on the therapy as 66 weeks. Total placebo exposure during the core EQUATOR study was 20 patient-years with a median time on placebo as 16 weeks. Treatment-emergent adverse events were mostly mild or moderate. There was 1 patient who reported a serious, non-fatal, drug-related treatment-emergent adverse event with filgotinib during the EQUATOR2 open-label extension study. One patient treated with filgotinib died on day 106 of the core EQUATOR study due to a serious drug-related treatment-emergent adverse event of pneumonia. There was a low incidence of treatment-emergent adverse events, except for respiratory tract infection which occurred at a rate of 46.2 per 100 patient-years of exposure. Decreased lymphocyte and neutrophil counts were the most notable abnormal changes in laboratory parameters with filgotinib treatment. At week 52 of the open-label extension, 81%, 55%, and 33% of patients achieved ACR20/50/70 responses. Response rates were maintained over time from open-label extension week 16 to week 52. At week 52, 34% of patients fulfilled criteria for minimal disease activity. Such rates were maintained over time from week 16 to week 52. The investigators noted the safety profile of filgotinib in patients with psoriatic arthritis was consistent with findings from rheumatoid arthritis studies with no new or unexpected safety signals. In EQUATOR2, the therapy was well-tolerated and showed sustained efficacy up to open-label extension week 52.
