Berenberg (PT € 225) GLPG1690 GLPG1690 the front runner among late stage, next generation IPF drugs. We believe the recent halting of Biogen’s BG00011 (STX-100) Phase IIb study places Galapagos in a race with FibroGen’s pamrevlumab, which recently began enrolling its Phase III study (ZEPHYRUS). Key opinion leader (KOL) diligence points to high enthusiasm for GLPG1690; the Phase III design is favorable; the dosing, a once daily pill, is preferred to both standard of care, which are dosed multiple times per day, and pamrev, which is dosed as an intravenous infusion. We raise our peak sales estimate from $1.5bn to $2.0bn owing to favorable competitive dynamics. We also raise our probability of success assumption to 60% (from 50%) following Gilead’s review of blinded safety data in ISABELA, which was part of its diligence for the expanded collaboration.If GLPG1690 is eventually approved for treatment of IPF, we estimate the value of GLPG1690 reach €50/share. As it is, we value GLPG1690 at €35 /share (vs. prior €30/share). GLPG1690 (IPF, Phase III) terms: Galapagos will receive an additional $325m milestone fee if GLPG1690 is approved in the U.S. for treatment of idiopathic pulmonary fibrosis, possibly in 2022, we estimate. Galapagos retains commercial rights for the EU on GLPG1690.H.C. Wainwright & Co (PT $205.00) Thoughts on GLPG1690. One potential concern voiced was that increased dosing of pirfenidone or nintedanib in the control arms could counter the decline in FVC leading to a diminished clinical effect associated with GLPG1690. However, this is unlikely given the tolerability issues with pirfenidone and nintedanib, and we expect higher than recommended dosing of these drugs could lead to increased rates of patient drop off in the control cohorts. FRI analysis from the FLORA study remains a key basis for management confidence in the program, but FRI remains a no-go in the Phase 3 ISABELLA programs due to cost and agency reticence. Note, validation for GLPG1690 comes from Bristol-Myers (BMY; not rated) decision to pursue a new molecule targeting LPA1, downstream of GLPG1690's target, and Boehringer Ingelheim's (private) decision to in-license an autotaxin program despite passing on GLPG1690 two years ago. Per management, the enrollment in the ISABELLA programs are now accelerating and is currently ahead of schedule.Goldman Sachs (PT € 108) GLPG1690 What’s new: We attended GLPG’s annual R&D Update event last week, with topics discussed largely as we expected. We learned a number of things that were new to us, but the key headline coming out of the event, in our view, was a delay in the timing of the highly anticipated interim (and futility) analysis of the Phase 3 data for GLPG1690 , an oral autotaxin inhibitor for the treatment of idiopathic pulmonary fibrosis (IPF) from early 2H20 to 1Q21 . And the potential of GLPG1690 in IPF: GLPG1690 is a small molecule inhibitor of autotaxin, which is the main source of LPA in the blood. Key takeaways on ‘1690 in our view, were: (1) the futility outcome is now expected in 1Q21 (vs. a prior summer 2020) due to a recalculation of the powering assumptions, representing a delay; (2) top-line Phase 3 data are expected in the early part of 2022; (3) the mechanism is believed to block LPA production and transport; (4) the autotaxin/LPA approach has been previously validated, via a Bristol-Myers Squibb (covered by Terence Flynn) candidate (BMS-986020), which ultimately lead to significant off-target toxicity, and therefore was discontinued.Svbleerink (Price Target: $194.00) We believe GLPG has a potential best-in-care drug in GLPG1690 that has the potential to become the standard of care in IPF treatment. With limited effective treatments in IPF, GLPG1690 carries blockbuster potential but with a projected 2023 market launch . Updates in IPF. GLGP’s pipeline also features a late stage IPF program led by the twin Phase 3 Isabela 1 and 2 trials for autotaxin (ATX) inhibitor GLPG1690, and followed up by the proof of concept (PoC) Phase 2 PINTA trial for GPR84 inhibitor GLPG1205. The LPA pathway was discovered as a driver of IPF in a masterstroke of translational research. Follow-up research to validate and therapeutically pursue this target builds on our conviction in GLPG1690. GLPG provided timing updates for a futility analysis for the Isabela trials, now guiding to the analysis in 1Q21 rather than 3Q20, but now with data from ~28% rather than 25% of the total patients. The Isabela trials continue to enroll rapidly , with ~1/3 of the trial (~500 patients) enrolled to date. Pending the outcome of the PINTA trial expected in 2H20, GLPG will next run a Phase 2b combination trial for GLPG1690 and GLPG1205 in IPF. GLPG is also running a Phase 2 PoC study for GLPG1690 in systemic scleroderma expected to read out in 2H20. GLPG1690 , filgotinib, and cash reserves comprise a majority of the valuation.
Credit Suisse (PT € 169) We also have lower expectations for ‘1690 in idiopathic pulmonary fibrosis (IPF) given the challenges in developing drugs for this indication, while ‘1972 in osteoarthritis (OA) is a highly competitive space. We await for key data updates in 2020 for both ‘1972 and ‘1690 before being more constructive. Bernstein (PT € 155) For GLPG1690 which will see no interim data but will confirm a go/no-go from the agency on trial progression. If the probability that the primary will not be met is small enough, the trial will continue. This is enough to be a major catalyst in our view. The product compares well vs. approved products. As we have previously stated, cross trials comparisons in IPF are a challenge. Even more so given the fact that the GLPG1690 study (i) was over a shorter period of only 12 weeks vs. +52-weeks for Esbriet/Ofev, (ii) only recruited 17 drug treated patients vs. hundreds for Esbriet/Ofev, (iii) recruited a slightly different patient population, (iv) had different endpoints (Exhibit 32). We have to take 17 patient data with a pinch of salt but looking at the 12-week data for both Esbriet and Ofev, (i) neither were able to demonstrate any form of improvement in FVC, which GLPG1690 did, (ii) both have inferior tolerability profiles, particularly GI, and (iii) both have inferior dosing schedules. GLPG1690 is a selective autotaxin (ATX) inhibitor. IPF is a severe, progressive lung disease marked by a highly variable clinical course which makes a confident diagnosis challenging to achieve. The track record of products in IPF is pretty poor and the only curative therapy for IPF remains lung transplantation. Drug treatment changed in 2014 with the approval of 2 drugs for the treatment of IPF (Esbriet and Ofev). However, tolerability is an issue, with substantial discontinuation rates for both medicines. In IPF, ATX levels rise in the broncheoalveolar fluid, and increased ATX activity has been detected in a range of inflammatory and fibroproliferative diseases in the lung, kidney and skin. ATX is the enzyme responsible for generating lysophosphatidic acid (LPA), with LPA being formed locally in areas with increased ATX levels and acting locally via its receptors. In the lung, LPA signalling via LPAR1, and possibly via LPAR2, activates G-protein-mediated signal transduction cascades (Exhibit 23). Apoptosis is triggered in epithelial cells, which in modelled pulmonary fibrosis is the initiating pathogenic event. Epithelial cells are also induced to secrete IL-8, which is both proinflammatory and stimulates permeability of endothelial cells, thus promoting pulmonary oedema. LPA has several effects on fibroblasts: it is a chemotactic factor that promotes fibroblast recruitment, while also being a stimulator of fibroblast activation (via TGF beta) and promotor of fibroblast survival. It should be noted that LPA signals through at least six receptors (including LPAR1 and LPAR2) which are expressed differentially across a wide range of tissues and with overlapping specificities. While GLPG1690 targets autotaxin (thereby reducing LPA production more generally, and reducing the effects of LPA through any of its receptors), LPAR1 is also being considered as a potential target for IPF treatment (e.g., BMS-986020 is an LPA receptor antagonist being developed by BMS that has completed p2 trials for IPF, although the data highlighted elevated liver enzymes and no active trials are on-going - link). The choice to target autotaxin might lead to unintended consequences; for example, LPAR2 is thought to protect against excessive innate immune responses to tissue injury, so targeting ATX might reduce this protective effect. However early clinical trials do not seem to suggest that GLPG1690 has unacceptably high rates of adverse events . The product compares well vs. approved products. As we have previously stated, cross trials comparisons in IPF are a challenge. With Galapagos owning full rights for the IPF portfolio, our peak sales estimates of €1.5B in 2030 (very realistic for an efficacious product, 2 existing products already +$1B 5 years in, detailed model in Exhibit 35) can be a big contributor to GLPG value (see our valuation analysis below). We would not call IPF a graveyard for drug development (we have better examples e.g. SLE) but given some patients may go periods of months with no worsening of disease, it will always be challenging to say with certainty that GLPG1690 will demonstrate superiority (hence our 30% probability of success). The initial data suggests the product should do well and given the complementarity to existing treatment, we would expect to see an additive benefit for patients. The way we see it, get an approval (late 2021/early 2022 launch) and the drug will sell.

Een gecombineerde fase 2b/3 studie GLPG1690/GLPG1205 zou nog beter zijn. Zal afhangen van data PINTA studie (GLPG1205). Verloopt introductie fase goed (fase 2b), kan je doorgaan met studie en overgaan op continuiteitsfase (fase 3). Dan heb je reeds vele, of alle, centra al open en kunnen patiënten die reeds gerekruteerd zijn doorgaan met behandeling. Dat zorgt voor een versnelling van marktintroductie bij verdere succesvolle data. SELECTION1 colitis ulcerosa fase 2b/3 studie is daar een voorbeeld van.

AMSTERDAM (AFN) - Biotechnologiebedrijf Galapagos heeft sneller dan verwacht de rekrutering van patiënten voor een zogeheten PINTA-studie inzake idiopathische pulmonaire fibrose afgerond. Die conclusie trekken analisten van KBC Securities. Daarmee loopt het onderzoek voor op schema. Volgens KBC biedt de studie een interessante update over de IPF-franchise van Galapagos. De proef is namelijk groot genoeg om een beeld te krijgen van de algemene werkzaamheid en de veiligheid van het middel. Dit in tegenstelling tot het traject van de fase II FLORA-studie waar momenteel 23 patiënten aan worden onderworpen, aldus KBC. Daar zal een vervolgstudie met 1500 patiënten meer duidelijkheid moeten verschaffen. Een tussentijdse uitlezing bij 30 procent van de patiënten in het fase III-programma wordt begin volgend jaar verwacht en blijft volgens KBC een risicovolle gebeurtenis voor het bedrijf. De kenners noemen het belangrijk dat een deel van het risico op het bordje van samenwerkingspartner Gilead is komen te liggen. In afwachting van verdere klinische uitlezingen, waarvan KBC er veel verwacht dit jaar, handhaaft de bank zijn hold-advies met een koersdoel van 189 euro.

Het is interessant om te lezen dat Walid vindt dat alles sneller is verlopen dan gepland. Daarnaast is het interessant om te lezen dat alle analisten dat statement zonder enig speurwerk klakkeloos overnemen. Vanaf januari 2019 heeft Galapagos in alle presentaties aangegeven dat de recrutering voor PINTA naar verwachting afgerond zou worden in Q4 2019. Volgens mij is Q4 2019 een kwartaaltje eerder dan Q1 2020. Laten we het er maar houden dat alles volgens plan is verlopen... en dat dat ook moge gaan gelden voor de resultaten! fc

Vanaf januari 2019 heeft Galapagos in alle presentaties aangegeven dat de recrutering voor PINTA naar verwachting afgerond zou worden in Q4 2019. Volgens mij is Q4 2019 een kwartaaltje eerder dan Q1 2020.
Laten we het er maar houden dat alles volgens plan is verlopen... en dat dat ook moge gaan gelden voor de resultaten!

Ik weet zelf niet goed wat Galapagos hoopt te bereiken met deze studie. Het doel is om 1205 te combineren met 1690 in een 2b studie. Verwacht G dat 1205 ook deze verschrikkelijke ziekte remt of moet dit vooral in combi gaan werken?
Galapagos koopt belang in Canadees biotechbedrijf JAN DE SCHAMPHELAERE Vandaag om 08:59Galapagos en Fibrocor Therapeutics breiden hun samenwerking uit en gaan samen op zoek naar nieuwe geneesmiddelen om fibrose te behandelen. Als onderdeel van het akkoord neemt Galapagos ook een aandelenbelang in het Canadese biotechbedrijf, en krijgt een zitje in de raad van bestuur. Details over het investeringsbedrag en het procentuele belang worden niet vrijgegeven. Een jaar geleden gingen de twee partijen al eens aan tafel zitten, wat uitmondde in een kleine samenwerking. Galapagos kocht daarbij de exclusieve rechten op een onderzoeksprogramma van Fibrocor naar een middel tegen IPF: idiopatische pulmonale fibrose, een chronische ziekte waarbij de longfunctie gestaag verslechtert door bindweefselvorming. Nu neemt Galapagos een optie om vier extra onderzoeksprogramma's - die nog in het labostadium zitten - naar zich toe te trekken. Galapagos betaalt daarvoor een niet gespecifieerde licentievergoeding, en zal extra geld op tafel leggen als het de opties licht. Daarna neemt Galapagos het onderzoek over en kan Fibrocor rekenen op mijlpaalbetalingen en royalties als het onderzoek opschiet.Speerpunt De deal toont aan dat Galapagos stilaan tot de groten behoort, en zijn pijplijn verder kan uitbouwen door interessante projecten in te kopen. Met een cashberg van 5,5 miljard euro (eind september) heeft het daarvoor alleszins geld voldoende. De keuze voor Fibrocor is niet geheel verrassend. De zoektocht naar IPF-middelen is voorbije jaren een van de speerpunten van Galapagos geworden. Een van de eigen IPF-middelen in ontwikkeling doorloopt momenteel de laatste studiefase waar 1.500 patiënten bij betrokken zijn. Daarnaast wil het ook fibrose (bindweefselvorming) in andere organen van naderbij bekijken. Fibrocor legt zich toe onder meer ook toe op fibrose van longen, nieren en lever, maar al het onderzoek zit nog in een vroeg stadium.'Fibrocor slaagt er duidelijk in met zijn technologieplatform nieuwe, interessante targets te identificeren', stelt Galapagos. Dat betekent dat Fibrocor de weg kan openen naar medicijnen die ziektes op een compleet nieuwe manier en via een nieuw werkingsmechanisme bestrijden. Het vertrekt daarbij van een uitgebreide biobank met niergegevens en een geneticaplatform waarmee het onderliggende genetische factoren van de ziekte blootlegt.
Bart zojuist tijdens JPM conference: Voor IPF Isabela nu 600 van de 1500 patienten geworven. Dat is dus 40%. Bij R&D dag (14 nov. 2019) was dat nog 30% (450 patienten). Gaat goed. In nov/dec/jan dus ruwweg 75 patienten/5% per maand erbij. In dit tempo nog ongeveer 12 maanden voor de werving van de laatste 60% (900 patienten), dus dan in Q1 2021 fullly enrolled, ongeveer tijdens gereedkomen van futility analysis dus.
