Its getting closer! :-) Currently my biggest worry is that the FDA and as a result the MRC (UK) won't lisence the drug that i am currently taking. However it looks like Gilead are planning to go to the FDA this year, which means for me that by the time i complete the clinical trial (I have 18mths left to go) I have a good chance of getting my Filgotinib on the NHS (UK). This drug has pretty much been a god send, I went from being in hospital and only being able to move on crutches to being self sufficent in 4 days. Best of all its an oral tablet and i have absolutely no side effects. Dont get me wrong, it's not prefect and i do take it in conjuntion with other meds (mtx, plaquenil, 2.5mg prednisone) but what a differnce, I am now almost almost at my pre RA self and i will take that as a massive win over this disease. So please take heart and stay strong, new drugs are always being developed, your perfect drug combination could be just around the corner!

Hoe denken jullie over de mogelijkheid van een black box label voor Abbvie's Upadacitinib ?
Voordat ik het Abbvie transcript las van de Goldman sachs conference was ik zelf vrij zeker van een black box label. Dit aangezien de safety resultaten uit de Upa studies vergelijkbaar zijn met Olumiant en Xeljanz, die beiden een black box hebben.
Bluf vanuit Abbvie of is het toch echt een reeele mogelijkheid dat upadacitinib geen black box warning krijgt?
Ook het feit dat Abbvie vrij zeker is dat er geen Adcom plaats gaat vinden (hiervan zullen ze half juni toch een redelijke inschatting van hebben kunnen maken), zou dit een teken aan de wand zijn?
Benieuwd naar de visies van anderen hierin. Alvast dank.
Abbvie transcript Goldman Sachs conference: seekingalpha.com/article/4269953-abbv... Is lastig dit te duiden F_VR. Zoals je zelf aangeeft: zowel geen AdCom als black box label is een mogelijkheid. AbbVie pretenteert dat upadacitinib een JAK1 selectieve remmer is, terwijl het duidelijk ook sterk JAK3 remt. Kans op infecties, gordelroos (herpes zoster) is daarmee groter dan een meer selectieve JAK1 remmer. De verschillen in JAK1 selectiviteit zijn onafhankelijk vastgesteld. Waarom heeft AbbVie geen label aangevraagd voor hogere dosis upadacitinib van 30mg per dag..? Eerst schermde AbbVie met hoge effectiviteit 12 mg 2x daags, welke haar gelijke zou vinden in de 30mg 1x daags. 30mg upadacitinib zou beter zijn dan 200 mg filgotinib. Hier hebben ze in zomer 2016 sterk mee geschermd met R&D presentatie. En eind 2018 laten ze 30mg dosis vallen, want 15mg volstaat. Dat 30mg een betere werkzaamheid geeft, moffelen ze weg. Dat 30mg safety issues geeft is overduidelijk. Ssst: niet over praten. Ratio met aantal gevallen diepe veneuze trombose, kans op longembolie, kans op malignancies: zeker bij 30mg is dit niet goed. Is ratio DVT/PE bij upadacitinib 15mg beter dan baricitinib 4mg..? AbbVie wil nu meeliften op succes van het veiligere Filgotinib. Upadacitinib is een 2e generatie JAK medicijn en noemt nu in 1 zinsnede daarbij ook Filgotinib. Wat de FDA gaat doen..? AbbVie is heel slim door de vergelijking met HUMIRA op te halen. Dit is echter maar 1 van de 5 SELECT studies. Kijk naar een SELECT BEYOND studie. Een matig veiligheidsprofiel van de 15mg dosis upadacitinib binnen deze studie. AbbVie wijt het aan background RA-patiënten. Gematigde tot ernstige vorm van RA is mede-oorzaak van bijwerkingen, mede door andere medicatie...? Cruciaal is upadacitinib dit effect versterkt. Op de LT zullen we meer weten. SELECT en FINCH-studies zijn niet 100% vergelijkbaar. Gender, aantal jaren RA, mate van ex-USA/EU patiënten en gemiddelde DAS28 (CRP) score bij aanvang studie. De toelatingscriteria en background medicijnen zijn echter wel nagenoeg gelijk. Dan vind ik Filgotinib op basis van FINCH 24-weeks en DARWIN 3 echt veel beter scoren dan Upadacitinib. Ter afsluiting kan nog worden gewezen op bijsluiter dat een medicijn meekrijgt bij markttoelating. Bij XELJANZ van Pfizer (JAK1/3 inhibitor) zijn de waarschuwingen niet van de lucht. Verhoogde kans op, bij, op, bij....www.xeljanz.com/ Hoe de goedgekeurde aanvraag markttoelating upadacitinib verder zal worden geduid door FDA weten we over week of 5.
Gilead gaat als ik het goed heb begrepen nog dit jaar goedkeuring voor Filgotinib aanvragen bij de FDA. Is er ook al bekend wanneer dit in Europa en Japan gaat gebeuren?
Er is augustus september genoemd voor EMA. Japan meen ik 4q gelezen ze hebben. Geen idee trouwens hoe lang de procedure in Japan duurt.
Galapagos krijgt een dikkere vinger in de pap omtrent strategische keuzes Filgotinib: "In het kader van de gewijzigde overeenkomst zal Galapagos een grotere rol krijgen bij de wereldwijde strategie van filgotinib..." Enkele acties die te verwachten zijn: » 52 weeks safety FINCH 1 & 3 komen in de publiciteit. Liefst voor Upadacitinib beslissing FDA 19/8. Mogelijk tegelijk met MAA-aanvraag bij EMA voor Filgotinib. » Voor nieuwe indicaties een fase 2/3 opstarten met Filgotinib. Denk aan Giant Cell Arteritis. » Snellere besluitvorming ontrent opstarten van fase 3 studie na goede fase 2 data. Zie PsA en AS (quote Onno 14/7: "fase 3 studies PsA en AS zijn gestart"). Zeer goed deze ontwikkeling. Binnenkort fase 2 data Syndroom van Sjogrens met Filgotinib. Gaan we zien hoeveel beter het paradepaardje van Galapagos is versus GS-9876 / GS-4059 / Placebo.

Wanneer precies zouden resultaten van Sjogren bekend worden ergens? (december 2019?). Sjogren is net zoals RA een gigantisch markt waar tot dusver zelfs huidige medicijn weinig helpen bij patiënten.
Zou toch in Q3 komen? Ik las ergens: “Eind juli verdere update, met hopelijk ook mooie fase 2 data van Syndroom van Sjogren's.”

Zeer vervelend dat je deze chronische ontstekingsziekten onder de lede hebt. Cross fingers dat Filgotinib wederom goede werkzaamheid laat zien in Syndroom van Sjogrens (SS). Er zijn meerdere studies gaande met diverse medicijnen teneinde werkzaamheid aan te tonen. Als Filgotinib goede data overlegt, zal het daarna (hopelijk) snel een fase 3 kunnen aanvangen voor SS met aanzienlijk minder patiënten dan een ziekte-indicatie als RA. Hopelijk kan je dan participeren alalush (België of Nederland).
Inderdaad. Sowieso ben ik niet van plan te wachten op terugbetalingsmodaliteiten voor Filgotinib. Eens het te koop is via de apotheek, koop ik het direct.

De Crohn studie loopt momenteel nog in 9 ziekenhuizen in Nederland. Weet niet of je daar aan mee mag doen vanwege her dubbele ziektebeeld, maar dat zou direct een "double punch" zijn om in de termen van Piet Wigerinck te blijven.
Avanti plaatste een geweldige analyse van Umer Raffat, analist voor Evercore ISI Biotech & Pharma Equity Research, op het Analyst report 2019 draadje. Heel veel dank voor het plaatsen! Umer geeft aan waarin Filgotinib zich chemisch onderscheid van de andere JAK's. Sheet 25: FDA has actually opined on a possible mechanistic link between JAK2 and platelet increases (and hence the DVT risk). Baricitinib was daarmee het eerste slachtoffer ten gevolge van het DVT risico. Vanaf sheet 29 gaat Umer in op de chemische overeenkomsten tussen de verschillende JAKs. Daarbij vermeld hij nadrukkelijk dat de geclaimde selectiviteit van een JAK geen absolute schaal is. Een kritische opmerking richting AbbVie dat altijd staat te beweren dat Upadacitinib een selectieve JAK1 remmer is!? En dan de verbindende factor van "alle" JAK's op sheet 32: if we look at chemical structures of most JAK inhibitors of interest, they all share one common theme: pyrrolo(2,3-d)pyrimidine ! Op sheet 35 (zie bijlage) staan de verschillende molecuul structuren van een twaalftal JAKs. Daar is duidelijk te zien dat het aangrijppunt van filgotinib geen pyrrole unit is, wat zich nadrukkelijk onderscheid van de belangrijke concurrenten Tofacitinib, Baricitinib en Upadacitinib. Dan een andere zeer belangrijke sheet 43: in een mooi overzicht vat hij de "cell-based assays data" van een aantal JAKs samen. Daar zie je duidelijk de geweldige data (selectiviteit) van Filgotinib staan (log schaal!). Als je naar die data kijkt vraag je je af hoe Upadacitinib een black-box waarschuwing kan ontgaan omdat de Upadacitinib data exact identiek is aan de data van Tofacitinib, die onlangs ook de black-box waarschuwing op hun medicijn kreeg! Daarmee is de beoordeling van het Upadacitinib dossier voor de FDA een geweldig dilemma... Het laatste stuk van Umer's presentatie is een lofzang op Filgotinib. Je zou hem bijna van partijdigheid gaan beschuldigen... Mijn eindconclusie: als er al een algemene trend zou zijn om alle JAKs van een black-box waarschuwing voor DVT te voorzien, maakt dit soort grondige analyses de kans op die algemene waarschuwing een stuk kleiner. fc

Dit kan wel eens een cruciaal moment worden zowel voor Abbvie als voor Gala/Gilead. Is er nog een kans dat de FDA om aanvullende onderzoeken gaat vragen bij Abbvie over upa?? Ik lees dit namelijk wel vaker bij een (toekomstig)middel waar nog niet alles helder is.

Uit de Abbvie call "What we've said consistently is our program hasn't demonstrated that risk. So if we were to pick up labeling language like that or warnings around this issue, that would have to come from a determination from the FDA that they're going to move from (edit, neem aan dat ze for hebben gezegd, niet nageluisterd) some form of class labeling." IMHO vies spelletje (er van uitgaande dat ze dit ook naar de FDA communiceren) van Abbvie. Als wij (abbvie) een label oppikken dan moet dat maar als class labeling. Even proberen om Filgo mee te zuigen in het eigen risico op een label. Misschien moeten O´Day en consorten zeer binnenkort breed uitdragen dat zij zeker geen class labeling voor de JAK´s verwachten gezien het safety profiel van Filgo. Er is geen class effect, net zoals er geen/bijna geen DVT achtergrond ruis is in RA patiënten. Nog meer reden lijkt me om de 52w data te publiceren. Helemaal geen slecht idee om de FDA een klein beetje gras voor de voeten weg te maaien, mochten ze in de verleiding komen.
Veiligheid van Filgotinib is de game-changer! Dat kan niet vaak genoeg herhaald worden. Als je de rest van je leven een medicijn moet slikken tegen een ziekte als RA, is "gebrek" aan bijwerkingen van het allerhoogste belang. Dan mag het zelfs nog een tikje slechter zijn (wat bij Filgotinib gelukkig niet/nauwelijks het geval is!). In the US rheumatoid arthritis (RA) market, it is clear that TNF inhibitors have historically dominated, with AbbVie's Humira and Amgen's Enbrel accounting for the lion's share of biologic/JAK-treated patients. However, as share for these agents are starting to dwindle, use of alternative mechanism of action (MOA) brands is anticipated to grow. Indeed, rheumatologists expect their share of Humira, Enbrel, and infliximab to drop significantly in the next six months while share of Regeneron's Kevzara and Eli Lilly's Olumiant are expected to do the opposite. Indeed, the majority of respondents do agree that CV risks are more likely a function of the JAK inhibitor class rather than isolated just to Xeljanz. However, when it comes to JAK and TNF inhibitor comparisons, though they are seen as equally efficacious, JAKs are perceived as far less safe.
