"Misschien" schreef terecht op het Mei draadje: Quote De pijplijn ligt niet stil, hooguit PE. Nierfalen wordt door COVID-19 patiënten ook duidelijk doordat door de infecties organen uit gaan vallen. De uitkomst van het onderzoek kon wel eens heel veel sneller gaan langs deze weg dan de weg van de pijplijn die tot nu toe gevolgd werd. Unquote Pharming heeft inderdaad de pijplijn vol met andere toepassingen van Ruconest, o.a. tegen acuut nier falen AKI. Hier is Dr. Michael Osthoff eveneens actief met onderzoek naar het gebruik van Ruconest. Hij heeft hier recent een publicatie op zijn naam. Methods In this placebo-controlled, double-blind, single-center trial 77 patients with chronic kidney disease were randomized to receive 50 IU/kg rhC1INH before and 4 h after elective coronary angiography or placebo. The primary outcome was the peak change of urinary neutrophil gelatinase-associated lipocalin within 48 h, a surrogate marker of kidney injury. Results Median peak change of urinary neutrophil gelatinase-associated lipocalin was lower in the rhC1INH group (4.7 ng/ml vs. 22.5 ng/ml; p = 0.038) in the per-protocol population but not in the modified intention-to-treat analysis, and in patients with percutaneous coronary interventions (median, 1.8 ng/ml vs. 26.2 ng/ml; p = 0.039 corresponding to a median proportion peak change of 11% vs. 205%; p = 0.002). The incidence of a cystatin C increase =10% within 24 h was lower in the rhC1INH group (16% vs. 33%; p = 0.045), whereas the frequency of contrast-associated acute kidney injury was comparable. Adverse events during a 3-month follow-up were similarly distributed. Conclusions Administration of rhC1INH before coronary angiography may attenuate renal injury as reflected by urinary neutrophil gelatinase-associated lipocalin and cystatin C. The safety profile of rhC1INH was favorable in a patient population with multiple comorbidities. Nog een interessant wetenschappelijk artikel betreffende de rol van AKI met betrekking tot Covid-19. Summary The outbreak of coronavirus disease 2019 (COVID-19) has rapidly evolved into a global pandemic. Most patients with COVID-19 have mild symptoms, but about 5% develop severe symptoms, which can include acute respiratory distress syndrome, septic shock, and multiple organ failure. Kidney involvement is frequent, with clinical presentation ranging from mild proteinuria to progressive acute kidney injury (AKI) necessitating renal replacement therapy (RRT). An understanding of the pathophysiology and mechanisms of kidney damage and AKI in the setting of critical illness and COVID-19 is emerging, although further research is needed to identify patients at risk of AKI and to guide management strategies. As no specific treatment options exist for AKI secondary to COVID-19, intensive care is largely supportive. Current approaches to prevention and management of AKI, and identification of potential indications for use of RRT and sequential extracorporeal therapies, are based mainly on clinical experience, and AKI strategies are adapted empirically to patients with COVID-19. International collaborative and cross-disciplinary research is needed to obtain adequate evidence to support current clinical approaches and to develop new approaches to management. Conclusions and future perspectives In the absence of specific anti-SARS-CoV-2 treatments, supportive care and use of sequential extracorporeal therapies for critically ill patients with evidence of kidney involvement provide a life support bridge to recovery and enhance the probability of a favourable outcome. The decision to use sequential extracorporeal therapies should take into account the technical effort and dedicated skills of the multidisciplinary staff that are needed for safe and effective therapy delivery. Careful patient selection for sequential extracorporeal therapies is necessary because age and comorbidities seem to influence outcomes in critically ill patients with COVID-19.22 Further research is needed to improve understanding of AKI secondary to COVID-19, to obtain adequate evidence to support the clinical approaches discussed here, and to develop new approaches to monitoring and management (panel). Fostering an international collaborative and cross-disciplinary research culture will be crucial to rigorously test therapies in clinical trials and to rapidly identify patients with COVID-19 who are at risk of AKI and who stand to benefit from established and emerging therapeutic approaches. Nog een opmerkelijke recent artikel betreffende Bradykinin en het ontstaan van Cytokine-stormen. The lungs are the major pathological target of SARS-CoV infection. The pathological derangements, aberrant signalling and dysregulated inflammation characteristics of lung cancer have marked similarities with the clinical manifestation of SARS-COV2 infection. As heightened inflammatory response is a key feature of both SARS-CoV infection and lung cancer and given the intriguing commonalities between the two, we analysed the RNA sequence dataset from lung cancer patients to get a better understanding of the critical regulators of inflammation in SAR-COV2. Methods: The Cancer Genome Atlas (TCGA) data sets of lung cancer were analyzed for correlations between ACE2, Bradykinin, and other genes associated with inflammatory responses. Results: TCGA dataset analysis indicated an inverse correlation between ACE2 and Bradykinin Receptor 1 (BDKRB1), IL-6 and IFNg in lung adenocarcinoma. A positive correlation between EGF, FOLR1, MAOA, NOSTRIN, MARC2, PHKD1, and ACE2 was noted Summary: Our analysis has identified a common (i) ACE2- BDKRB1-inflammatory network and (ii) FOLR1-Nitric oxide cross-talk in lung cancer and SARS-CoV2. Better understanding of the convergences between critical players in these networks would enable efficient repurposing of approved lung cancer therapeutics for COVID-19 infection. Conclusion: As SARS-CoV2 infections are characterised by an aberrant inflammatory response, a combinations of anti-viral and anti-inflammatory has been suggested for reducing viral infectivity and dysregulated host inflammatory response [59]. With the pandemic human pathogenic coronavirus disease 2019 (COVID-19) spreading unabated, the current challenges are to identify therapeutic strategies and efficiently evaluate their activity in clinical setting. This comparative analysis has highlighted that functional integration of diverse intracellular signalling networks in lung cancer can be extrapolated to explain the aberrant inflammatory response in COVID-19 infection. Our finding suggests that therapeutics with safety and tolerability profiles in lung cancers targeting BDKRB1 and FOLR1 could be repurposed for calming the cytokine storm in COVID-19 infection. Unquote Dit soort overeenkomsten biedt duidelijk een nieuw perspectief voor de pijplijn van Pharming. Immers, indien Ruconest succesvol blijkt bij het tegen gaan van Cytokyne stormen als gevolg van een Covid-19 infectie, lijkt een onderzoek naar het effect van Ruconest als potentieel longkanker medicijn niet onlogisch. Ton, De KISS benadering staat natuurlijk voor Keep It Simple Stupid. De laatste S is natuurlijk een zelf reflectie van Jesse. Maar je hebt gelijk met 2 vliegen in één klap. Ruconest werkt gunstig voor het kalmeren van een Cytokine storm wat kan lijden tot ARDS (Acute Respitary Distress Syndrome) of te wel het voorkomen van overtollig vocht in de longen. Maar Ruconest werkt ook gunstig ter voorkoming van AKI oftewel acute nierfalen, hierover heeft Dr Osthof recent een voor Pharming een gunstige publicatie geplaatst. Stukje uit de column van Errol Keyner Remuneratievoorstel van de agenda Gelukkig maar, want Pharming had net het agendapunt over de beloning van de agenda gehaald en de aandeelhoudersvergadering van Pharming is gebaat bij corona-vaccin (VEB)VEB wilde meer weten over de beweegredenen. Met wat vertraging kwam het antwoord dat er niet meer zou worden gemeld dan eerder in de vergadering, waarna de oude verklaring nogmaals werd voorgelezen, die ik ditmaal direct in het Engels kon volgen. . Met enige krachtsinspanningen vertaald: “Diverse buitenlandse institutionele beleggers en de stemadviesbureaus erkennen niet de unieke concurrerende status van Pharming als een Nederlands bedrijf dat concurrerend moet zijn in de Amerikaanse markt”. But these are very expensive medications and difficult to produce rapidly in quantities.
