European Medicines Agency maintains recommendation not to grant a marketing authorisation for Glybera 20/04/2012 Insufficient evidence to show benefit of Glybera in lipoprotein lipase deficiency patients with severe or multiple pancreatitis attacks The European Medicines Agency’s Committee for Medicinal Products for Human Use (CHMP) at its April 2012 meeting maintained its recommendation not to grant a marketing authorisation for the orphan medicine Glybera (alipogene tiparvovec), from Amsterdam Molecular Therapeutics B.V. This followed a request of the European Commission in January 2012 to review the benefit risk of Glybera in a restricted group of patients with severe or multiple pancreatitis attacks. Glybera is a gene-therapy product using an adeno-associated viral vector and was intended for the treatment of adult patients diagnosed with lipoprotein lipase deficiency demonstrating hyperchylomicronaemia or having a history of acute pancreatitis. When evaluating Glybera in patients with severe or multiple pancreatitis attacks, the CHMP concluded that the evidence that Glybera reduced pancreatitis attacks in the small number of patients assessed (data from only 12 patients were available) was not sufficiently convincing. In addition, the reduced risk of pancreatitis seen in a few of the patients could have been due to other factors (such as changes in lifestyle and diet, and the natural course of the disease). In its discussions, the Committee recognised the difficulty of obtaining and assessing data in this very rare disease. However, after careful consideration of all the evidence and circumstances of the disease, the Committee maintained its previous recommendation not to grant a marketing authorisation. Refusal of the marketing authorisation for Glybera (alipogene tiparvovec) On 23 June 2011, the Committee for Medicinal Products for Human Use (CHMP) adopted a negative opinion, recommending the refusal of the marketing authorisation for the medicinal product Glybera, intended for use in patients with lipoprotein lipase deficiency. The negative opinion was confirmed in October 2011, following a re-examination requested by the company, Amsterdam Molecular Therapeutics (AMT) B.V. Following a request from the European Commission in January 2012, the CHMP re-evaluated Glybera in a restricted group of patients with severe or multiple pancreatitis attacks. The CHMP maintained its previous recommendation that the medicine should not be granted marketing authorisation. What is Glybera? Glybera is a medicine that contains the active substance alipogene tiparvovec. It was to be available as a solution for injection. Glybera was developed as a type of advanced therapy medicine called a ‘gene therapy product’. This is a type of medicine that works by delivering a gene into the body to correct a genetic deficiency. What was Glybera expected to be used for? Glybera was expected to be used to treat lipoprotein lipase deficiency, a very rare disease where patients lack the gene to produce lipoprotein lipase, an enzyme responsible for breaking down fats in lipoproteins (fat-carrying particles in the blood). Glybera was designated an ‘orphan medicine’ (a medicine to be used in rare diseases) on 8 March 2004 for treatment of lipoprotein lipase deficiency. How is Glybera expected to work? The active substance in Glybera, alipogene tiparvovec, is a medicinal product using a virus that has been modified so it can carry a gene for lipoprotein lipase. It was expected to be injected into the patient’s muscles, where it would correct the deficiency of lipoprotein lipase by enabling cells of the muscle to produce the enzyme. The virus used in Glybera is an ‘adeno-associated viral vector’ that has been modified so that it cannot make copies of itself. The virus does not cause infections in humans. What did the company present to support its application? The effects of Glybera were first tested in experimental models before being studied in humans. The company’s clinical programme included 27 patients with lipoprotein lipase deficiency on a low-fat diet. The majority of patients who received Glybera also received immunosuppressive treatment to reduce the reactions of the body’s immune system against medicine. The main measure of effectiveness was based on a reduction in blood fat levels, with patients being followed up after the study. What were the CHMP’s main concerns that led to the refusal? Because Glybera is an advanced therapy medicine, it was also assessed by the Committee for Advanced Therapies (CAT). Taking into account the initial negative opinion provided by the CAT, the CHMP concluded at its initial assessment that the studies had not shown a consistent long-lasting benefit of Glybera. The applicant had not provided sufficient evidence of a persistence of effect in lowering blood fats in a clinically relevant manner and there were too few patients for whom sufficiently long-term data were available. There was also insufficient evidence of a reduction in the rate of pancreatitis (inflammation of the pancreas), which is a major complication of lipoprotein lipase deficiency. In view of the uncertainty over Glybera's benefits, the CHMP was concerned that there was insufficient evidence that its benefits outweigh its potential risks. During the re-examination, the CAT concluded that these concerns could be addressed with additional post-marketing studies. Whilst the CHMP considered Glybera to be potentially valuable in the treatment of this very rare disease, it took a different view and concluded that the benefits of the medicine did not outweigh its risks due to questions over the medicine’s benefits. The initial recommendation that Glybera should not be granted marketing authorisation was therefore maintained. When evaluating Glybera in patients with severe or multiple pancreatitis attacks, the CHMP found that the evidence of efficacy from the small number of patients assessed (data from only 12 patients were available) was not sufficiently convincing. Mooi kut!! Zou het een machtsspel zijn tussen CHMP en EC? Zal de EC nog wat doen? Zou mooi zijn als ze hun ballen laten zien, maar ik ben bang dat het hier bij blijft.
Een hemofilie B–partnerschap zal de deur(vh kooike) dienen te openen (eventueel samen met het GDNF&AIP programma en Sanfilippo B met Inst. Pasteur).

Het is niet anders. Wat denk jij... 1) In hoeverre zou Forbion uit zijn gegaan van succes Glybera? 2) Verwacht je nog een vervolg op Glybera? EC dat toch goedkeuring onder bijzondere omstandigheden verleent of Glybera partnerschap buiten EU?
waarom kwam dat bericht over goede resultaten mbt glybera net voordat amt van de beurs ging? waarom dan nu dit bericht nadat amt juist van de beurs is? volgens mij is er iets gaande en seinen ze dat het 'goed zit'....maar ja, dat is mijn gevoel. Het lijkt georkestreerd.

Het positieve is dat ze dat toch al eens eerder is gelukt? Weet jij zo welke transactie dat was? En voor hoeveel denk je dat ze uniQure weten te slijten?

(stand 17 tegen, 16 voor)Beslissing wordt uitgesteld, nieuwe gegevens gevraagd en gegeven. Ondertussen gaat amt van de beurs. Nu laat CHMP weten nog steeds negatief te adviseren. 23 juni a.s. (geloof ik) definitieve beslissing van eur.com. Maar, zoals chmp advies naast zich neerlegde, kan de ec dit nu ook doen met advies van chmp. (Het blijft tenslotte advies). Ik hoop, zoals al eens geopperd is, op een gedeeltelijke goedkeuring. (Per geval bekijken.)

Maar, zoals chmp advies naast zich neerlegde, kan de ec dit nu ook doen met advies van chmp. (Het blijft tenslotte advies). Ik hoop, zoals al eens geopperd is, op een gedeeltelijke goedkeuring. (Per geval bekijken.)
Dat help ik je mee hopen. Maar de keuze voor gedeeltelijke goedkeuring zou wel eens van de CHMP moeten komen en niet van EC. De vraag is waarom CHMP die ruimte niet ziet, wil zien of mag zien.
Heeft misschien te maken met het feit dat bij goedkeuring in europa, het traject in amerika niet meer hoeft te worden doorlopen. Ik weet niet of dit bij dit medicijn ook het geval is, er zijn uitzonderingen. Het zou echter heel gunstig kunnen zijn voor een amerikaanse investeerder, die niet mag investeren in beursgenoteerde bedrijven.
Goedkeuring Glybera is direct succes en doorgang volgende fase, maar volgens mij is de ontwikkeling in hemofilie B voor de toekomst eentje die grotere impact heeft voor eventuele doorverkoop UniQyre..
En daar zijn al gesprekken over gaande.....hoorde iemand op de ava.
Dit plaatste Franky Tornado na de ava: Hi Guys, Ja ik ben er geweest gisteren. Er was een menigte waar we mee aan het vergaderen waren, dus regelmatig orde problemen (LOL). Er waren dus 3 personen, een 4e was verwacht, maar niet op komen dagen. Ik hoop dat er de 30e wat meer volk is. Wel goed kunnen praten met Verdonk, Aldag en Morgan. Alles in een open sfeer. Er komt een verslag van het geheel op de AMT website. Geen grote nieuwe ontwikkelingen, behalve dan, maar dat is een opmerking a titre personnel en mijn interpretatie, dat ik in de ogen van Aldag iets kon lezen van een positieve ontwikkeling op het front van samenwerkingsverbanden. De €1 mln, die nog geregeld moeten worden is in elk geval geen probleem meer. Meer wilde hij hier niet over kwijt. Wel zei hij dat het de bedoeling is van Forbion om er met een termijn van 1,5 - 3 jaar uit te stappen. Hetzij via een overname, hetzij via een IPO. Hi Guys, Ik heb er nog steeds vertrouwen in. Dat hele Glybera verhaal is een mooie oefening geweest voor het bedrijf (AMT/uniQure). De volgende keer zal dit niet meer gebeuren. de gepubliceerde resultaten zijn dusdanig goed voor het hemofilieprogramma, dat men hier niet meer aan voorbij kan gaan. De transistor is ook pas tijdens de tweede wereldoorlog in gebruik genomen. Puur omdat de buizenfabrikanten de boel tegenhielden. Omdat de vliegtuigen een goede radiovoorziening moesten hebben zijn de transistoren eindelijk in gebruik genomen. Met de gen therapie is het gewoon eenzelfde soort verhaal. De mensen in de commissies moeten terzake deskundig zijn en zijn dus allen met een farma-achtergrond. De grote farma gaan een gevoelige klap krijgen wanneer de gen therapie van start gaat. Die $70.000 per jaar voor een hemofilie patient gaan dan aan hun neus voorbij. En zo zijnn er nog een hele reeks meer ziekten, die nu een leuke melkkoe zijn voor de farma-boys. En die ga je natuurlijk niet zomaar naar de slager brengen. Er moet dus een onafhankelijke commissie komen voor dit soort therapien. Maar zoals gezegd: Ik hou er vertrouwen in. :) Frank

Oh oh oh. AMT investeerde 100 miljoen (!) in een therapie dat zich niet laat terugverdienen omdat het niet toegepast mag worden. AMT is het volgende slachtoffer nadat eerdere aanvragen (van andere bedrijven) het ook niet redde in de aanvraagprocedure. Het een leuke oefening noemen is wel erg gemakzuchtig. De EC moet de rug recht houden en AMT toegang verlenen tot de bijzondere goedkeuring. Hiermee krijgt de EU ook haar subsidiegeld indirect terug.
Three Strikes for Glybera, AMT Most Likely Out of Business LONDON – The European Medicines Agency (EMA) has given a thumbs down to the gene therapy Glybera (alipogene tiparvovec) for the third time after being asked by the European Commission to think again about its refusal in January to recommend approval of the treatment for the inherited disorder lipoprotein lipase deficiency (LPL). That created an awkward precedent, since it was the only occasion on which the European Commission had failed to rubber stamp a recommendation from the EMA's Committee for Human Medicinal Products (CHMP) on whether to approve or reject a drug. The CHMP announced on Friday that it is sticking to its guns and upholding the recommendation not to grant a marketing authorization for Glybera. While the decision means there is still no approved gene therapy treatment in a regulated market, the CHMP has not rejected Glybera because of any safety concerns. Rather it said there was not convincing proof of efficacy that it prevented recurrent pancreatitis, the main symptom of the disorder. LPL is an ultra-orphan disease and the file contains data from 12 patients only. The rejection is likely to mean the end of the line both for the product, and for its developer Amsterdam Molecular Therapeutics BV (AMT). Last month the company was taken private by one its biggest investors, Forbion Capital Partners, securing AMT's immediate future, while it awaited the outcome of the CHMP's third review of Glybera. But the cash is due to run out this month. (See BioWorld Today, Feb. 22, 2012.) In a statement, the CHMP said it had concluded the evidence that Glybera reduced pancreatitis attacks in the small number of patients assessed was not sufficiently convincing. "In addition, the reduced risk of pancreatitis seen in a few of the patients could have been due to other factors, such as changes in lifestyle and diet, and the natural course of the disease," the statement said. The CHMP acknowledged the difficulty of obtaining and assessing data, but noted, "after careful consideration of all the evidence and circumstances of the disease," it was upholding its previous recommendation not to grant a marketing authorization. AMT contended that Glybera offers sufficient clinical benefit to justify a conditional approval, with the proviso that patients who have received the one-off gene therapy treatment continue to be followed. The company agreed to maintain a patient registry and to follow a risk management plan under which no more than two patients per month would be treated at a limited number of expert centers. But now CHMP has insisted yet again that the evidence base is too thin to justify approval. The committee recommended the refusal of a marketing authorization for Glybera in June 2011 , confirming its negative opinion in October 2011, following a re-examination requested by AMT. AMT has lobbied long and hard to push Glybera over the line, claiming the CHMP is at odds with other experts on the question of clinical benefit. In particular, EMA's Committee for Advanced Therapies, set up to give scientific advice on gene and cell therapies, recommended Glybera be approved. AMT's CEO Jorn Aldag has also argued the CHMP has not followed the EU's rules for approving orphan drugs, which dictates that if controlled trials leading to statistical significance cannot be conducted, individual (narrative) patient data should be weighed in assessing possible benefit. The venture capital investors in AMT have been keeping the company afloat in the hopes that Glybera would be approved. In addition to giving AMT the status of having a marketed product the signal that regulators will approve gene therapy products was expected to provide the validation AMT needed to close a partnership deal for its second product, a gene therapy treatment for hemophilia B.
