(ABM FN-Dow Jones) Virusremmer remdesivir van Gilead Sciences heeft in de Verenigde Staten opnieuw positieve resultaten laten zien als behandeling tegen Covid-19. Dit meldde het National Institute of Allergy and Infectious Diseases vrijdagavond laat. Nieuwe data laten zien dat patiënten die remdesivir kregen toegediend, het ziekenhuis na 11 dagen konden verlaten. Bij patiënten in een placebo-groep volgde ontslag uit het ziekenhuis na 14 dagen. Ook de sterftegraad daalde bij de groep die remdesivir kreeg toegediend, volgens NIAID. Eind april maakte Gilead al bekend dat patiënten met ernstige Covid-19 dezelfde verbetering vertoonden wanneer ze remdesivir vijf dagen kregen toegediend als ernstig zieke patiënten die tien dagen werden behandeld met het middel. De resultaten van die studie waren vooral bedoeld om de toedieningsperiode van vijf en tien dagen met elkaar te vergelijken, aldus Gilead. Bij een kortere kuur zouden mogelijk meer mensen het middel toegediend kunnen krijgen. Een test met remdesivir in China leverde eerder tegenvallende resultaten op. Dat kwam volgens Gilead omdat die studie vroegtijdig werd stopgezet. De Amerikaanse Food and Drug Administration heeft Gilead inmiddels goedkeuring gegeven om het middel te gebruiken als noodbehandeling bij patiënten die besmet zijn met het coronavirus. Ook zijn er deals gesloten om remdesivir wereldwijd te kunnen distribueren.

WHO staakt onderzoek naar malariamedicijn als middel tegen COVID-19 De gezondheidsorganisatie WHO heeft de klinische proef met hydroxychloroquine als middel tegen COVID-19 opgeschort vanwege twijfels over de veiligheid van het malariamedicijn, maakt ze maandag bekend. De proeven zijn tijdelijk gestaakt nadat het medische tijdschrift The Lancet onlangs een onderzoek publiceerde dat aantoonde dat het medicijn verband hield met een verhoogd risico op overlijden of hartaandoeningen. "Het is belangrijk om nieuwe bewijzen over de werkzaamheid en veiligheid van hydroxychloroquine te blijven verzamelen", zei hoofdwetenschapper Soumya Swaminathan van de WHO maandag tijdens een persconferentie in Genève. "We willen het middel alleen gebruiken als het veilig en effectief is, het dodental verkleint en de opnameduur verkort, zonder de bijwerkingen te vergroten", zei ze. Gilead Announces Results From Phase 3 Trial of Remdesivir in Patients With Moderate COVID-19 June 1, 2020- Study Demonstrates 5-Day Treatment Course of Remdesivir Resulted in Significantly Greater Clinical Improvement Versus Treatment with Standard of Care Alone - Data Add to Body of Evidence from Prior Studies Demonstrating Benefit of Remdesivir in Hospitalized Patients with COVID-19 Gilead Sciences, Inc. (Nasdaq: GILD) today announced topline results from the Phase 3 SIMPLE trial in hospitalized patients with moderate COVID-19 pneumonia. This open-label study evaluated 5-day and 10-day courses of the investigational antiviral remdesivir plus standard of care, versus standard of care alone. The study demonstrated that patients in the 5-day remdesivir treatment group were 65 percent more likely to have clinical improvement at Day 11 compared with those in the standard of care group (OR 1.65 [95% CI 1.09-2.48]; p=0.017). The odds of improvement in clinical status with the 10-day treatment course of remdesivir versus standard of care were also favorable, trending toward but not reaching statistical significance (OR 1.31 [95% CI 0.88-1.95]; p=0.18). No new safety signals were identified with remdesivir across either treatment group. Gilead plans to submit the full data for publication in a peer-reviewed journal in the coming weeks. "Our understanding of the spectrum of SARS-CoV-2 infection severity and presentations of COVID-19 continues to evolve," said Francisco Marty, MD, an infectious diseases physician at Brigham and Women’s Hospital, and associate professor of medicine at Harvard Medical School. "These study results offer additional encouraging data for remdesivir, showing that if we can intervene earlier in the disease process with a 5-day treatment course, we can significantly improve clinical outcomes for these patients." Remdesivir is currently approved in Japan as a treatment for patients infected with SARS-CoV-2, the virus that causes COVID-19. Outside of Japan, remdesivir is an investigational, unapproved drug. The U.S. Food and Drug Administration (FDA) granted remdesivir an Emergency Use Authorization for the treatment of hospitalized patients with severe COVID-19; the authorization is temporary and does not take the place of the formal new drug application submission, review and approval process. In this study, hospitalized patients with confirmed COVID-19 infection and evidence of pneumonia without reduced oxygen levels were randomized (1:1:1) to receive open-label remdesivir for 5 or 10 days or standard of care alone. The primary endpoint was the clinical status as assessed by a 7-point ordinal score at Day 11, ranging from hospital discharge to increasing levels of oxygen and ventilatory support to death. The secondary study objective was the rate of adverse events in each remdesivir treatment group compared with standard of care. At Day 11, a higher proportion of patients in the 5-day treatment group achieved improvement in clinical status versus the standard of care group, achieving statistical significance for a = 1-point improvement in ordinal scale (p=0.026). In addition, non-statistically significant increases in clinical worsening or death were observed in the standard of care only group compared with the remdesivir groups. "We now have three randomized, controlled clinical trials demonstrating that remdesivir improved clinical outcomes by several different measures. Today’s results showed that when treating moderate disease, a 5-day course of remdesivir led to greater clinical improvement than standard of care, adding further evidence of remdesivir’s benefit to previously released study results. The National Institute of Allergy and Infectious Diseases’ placebo-controlled study showed that remdesivir enabled more rapid recovery and that earlier treatment improved clinical outcomes. Our SIMPLE-Severe study showed that when treating patients with severe disease, 5 days of remdesivir led to similar clinical improvements as a 10-day course," said Merdad Parsey, MD, PhD, Chief Medical Officer, Gilead Sciences. "The additional data we have in hand today will further guide our research efforts, including evaluating treatment earlier in the course of disease, combination studies with other therapies for the most critically ill patients, pediatric studies and the development of alternate formulations." Remdesivir was generally well-tolerated in both the 5-day and 10-day treatment groups. House, SA News Editor The European Medicines Agency (EMA) reports that Gilead Sciences (GILD +0.6%) has submitted its marketing application seeking conditional approval of antiviral remdesivir for the treatment of COVID-19. Considering that the regulator has already reviewed quality and manufacturing data, preliminary results from several clinical trial and supporting data from compassionate use programs, the review timeline will be shortened and the advisory group CHMP may issue an opinion "within weeks." If positive, the EMA will work closely with the European Commission to fast track a final decision. Remdesivir is currently approved for emergency use in the U.S. and Japan. The nod in Australia is imminent after a taskforce backed approval last week.

De studie toont aan dat de geïnfecteerde apen die het middel kregen toegediend, geen tekenen vertoonden van een luchtwegaandoening en ook minder schade hadden aan de longen, aldus de auteurs van het artikel. Eerder deze maand werd bekend dat het coronamedicijn remdesivir in een grootschalige studie slechts in beperkte mate wist te overtuigen. De Fase 3 studie toonde bij coronapatiënten met gematigde klachten die het middel 5 dagen kregen toegediend verbetering ten opzichte van patiënten die op normale wijze werden behandeld. Deze patiënten hebben een kans van 65 procent dat ze op dag 11 meer verbetering laten zien dan regulier behandelde patiënten. House, SA News Editor India's Controller General has approved generic versions of Gilead Sciences' (NASDAQ:GILD) remdesivir for the treatment of COVID-19. Two domestic drugmakers, Hyderabad-based Hetero and Mumbai-based Cipla, plan to launch their offerings within a week at Rs 5,000-6,000 ($65-78) and Rs 3,000-4,000 ($39-52), respectively. According to current Indian government guidelines, a COVID-19 patient may need five-to-six doses, implying a total treatment cost of the antiviral as low as $195. Gilead has yet to announce the price in the U.S. but it's safe to say it will be substantially higher. Drug cost watchdog ICER issued a report last month that the medicine would be cost-effective at $4,500 per course of treatment.

Op basis van de aanbeveling van de toezichthouder zal de Europese Commissie het gebruik van het middel vermoedelijk komende week goedkeuren.
